commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; quotations
are being requested and a written solicitation will not be issued.
Solicitation number SSA-RFQ-99-0004 is issued as a Request For
Quotation (RFQ). This solicitation document and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
97-08. This acquisition is a 100% small business set-aside. Quotations
submitted by concerns other than small businesses will not be
considered. The following provisions and/or clauses apply to this
solicitation: 52.212-1, 52.212-3, 52.212-4 and 52.212-5. The Social
Security Administration, Harold Washington Social Security Center, 600
W. Madison Street, Chicago, IL 60661, requires the purchase and
installation of a warehouse security storage cage. Minimum requirements
for the security storage cage are as follows: 10 gauge wire mesh
partitions, 4.5 meters wide (approx. 15') x 3.0 meters long (approx.
10') x 3.0 meters high (approx. 10'), to include one 1.5 meter wide
(approx. 5') x 2.4 meter high (approx. 8') sliding door and 13.5 square
meters (approx. 150 sq ft) of wire mesh ceiling. All wire mesh
partition surfaces will have a painted finish (colors to be identified
by offeror). The security storage cage will be installed as a corner
unit against 2 existing concrete walls. The linear length of the wire
mesh partitions will be a total of 7.5 meters (approx. 25') x 3.0
meters high (approx. 10'). Offeror must provide price quotations as
follows: (1) price for storage cage components (wire mesh partitions,
sliding door, ceiling panel, bolts, screws, etc.); and (2) price for
installation of the security storage cage. Successful contractor will
be required to remove all packaging material and installation debris
from the site. All prices quoted must be FOB Destination. Offeros must
provide estimated delivery and installation date(s). Award will be
made to the vendor offering the best overall value, taking into
consideration price and delivery/installation date(s). All offers must
include a completed copy of the provisions at 52.212-3, Offeror
Representations and Certifications -- Commercial Items. Failure to
include a completed copy of 52.212-3 may result in rejection of an
offer. Offers will be accepted either in writing, at the address stated
above, or by facsimile, at 312-575-4156, attn: Nestor Floresca. Please
reference solicitation number SSA-RFQ-99-0004 on all offers. All
offers are due no later than 3:00 pm (Chicago time) on Thursday,
