(850)884-1258 This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in Federal Acquisition
Regulations (FAR) subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. This is request for quotation (RFP)
F08620-99-T0011 will be based on a Statement of Objective. The RFP
incorporates provisions and clauses as those in effect through Federal
Acquisition Circular Number 97-09. The following (FARs) provisions and
clauses apply to this solicitation and are incorporated by reference:
FAR 52.212-1, Instructions to Offerors-Commercial Items; FAR 52.212-2,
Evaluation-Commercial Items; FAR 52.212-3, Offeror Representations and
Certifications-Commercial Items; FAR 52.212-4, Contract Terms and
Conditions -- Commercial Items and FAR 52.212-5, Contract Terms and
Conditions Required to Implement Statutes or Executive Orders. FAR
52.212-5 is completed so that the following clauses under paragraph (b)
apply: 6, 7, 8, and 9; (c) apply: 1 (WD 94-2121, Rev 12 is hereby
incorporated),2. and FAR 52.245-2 -- Government Property (Fixed Price
Contracts) (Dec 1989) ALT 1, (Deviation) APR 1984. The following
Defense FAR Supplements (DFARS) provisions and clauses apply to this
solicitation and are incorporated by reference: DFARS 252.212-7000,
Offeror Representations and Certifications-Commercial Items; and DFARS
252.212-7001, Contract terms and Conditions Required to Implement
Statutes or Executive Orders Applicable to Defense Acquisitions of
Commercial Items. The following clause under paragraph (b) of
252.212-7001 applies: DFARS 252.225-7001. FAR 52.212-1 is amended to
read: "Submit signed and dated offers in three copies to 16 CONS/LGCY,
P.O. Box 9190, 350 Tully St (Mail Stop #39), Hurlburt Field, Fl
32544-9190. Offers may be submitted on letterhead stationery or a SF
1449 and at a minimum must show: (1) The name, address, and telephone
number of the offeror; (2) delivery time; (3) price and discount terms;
(4) representations and Certifications at FAR 52.212-3 and DFARS
252.212-7000; and (5) if the offer is not submitted on SF 1449, include
a statement specifying the extent of agreement with all terms and
conditions, and provisions included in the solicitation. An official
authorized to bind your organization must sign the proposal. Offers
that fail to furnish the required representations and certifications,
or reject the terms and conditions of the solicitation, may be excluded
from consideration. FAR 52.212-2 Evaluation-Commercial Items, is
amended to read: "The Government will award a contract resulting from
this solicitation to the responsible offeror whose offer conforming to
the solicitation will be most advantageous to the government, price
and other factors considered. The government anticipates award of a
firm-fixed price purchase order. The following factors shall be used to
evaluate offers: (1) Price, (2) Statement of Work, (3) Past
Performance." Questions concerning this RFP or requests for government
provided equipment listing and map must be submitted in writing; no
telephonic responses will be processed. Questions should be faxed to
Twila Beasley and addressed to 16 CONS/LGCY at (850) 884-5372 to arrive
not later than 20 Nov 98. A contractor developed Statement of Work
shall be part of your offer based on the following objective: The
contractor is to purchase and install seven class one telephone poles
(50 Ft); install government provided Whelen Co sirens (equipment);
connect power to the Electronic Cabinets, Motorola equipment, and
Whelen equipment according to manufacturer's specifications. Install
all provided Motorola equipment (must be installed by an authorized
Motorola Rep) according to manufacturer's installation specifications;
Contractor will coordinate all work with supporting organizations.
After installation, test equipment to ensure it meets all
manufacturer's operational specifications. Also, a list of the last
five contracts performed by your company to include valid phone numbers
and points of contact. The job will be considered complete when the
system is operational. The anticipated value of this project is less
than $40,000.00. The planned start date is on or about 5 Jan 99. FOB is
Destination. Offers must be received by 2 Dec 98, 3:00 p.m. central
time at 16 CONS/LGCY, P.O. Box 9190, 350 Tully St (Mail Stop #39),
