The VA Medical Center, Prescott, AZ intends to issue a solicitation for Community Based outpatient Care Services (Primary Care)(CBOC). Contractor is to provide Primary Care services to eligible Veterans residing in the Coconino County portion of the primary service area of the Department of Veterans Affairs Medical Center Prescott Arizona, for a period of one year from the date of the Notice to Proceed. Veterans to be provided services under this capitated rate contract, reside primarily in Flagstaff Arizona and the surrounding areas of Coconino Country. The contract expected to be awarded from this solicitation will include a Base Year and two (2) one year renewal options which may be exercised at the option of the Government.
