Clark Street, Chicago, IL 60605 as described below: Model No. 9001/S -- four (4) drives to be rebuilt with Upgrade 2 (Optics kit, AMG, TDF, AZR, Proms); Model No. 9001/CS -- one (1) with Upgrade 2 (Optics kit, AMG, TDF, AZR, Proms); Model No. 9001/CS -- four (4) with Upgrade 5 (Optics kit, AMG, Ctl Prom, Spindle Motor). The above drives are manufactured by ATG in France and TAB Products is the only known U.S. company authorized to repair or rebuild these drives.
