COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART
12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS
NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE
BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This
solicitation is issued as Request for Quotation No. RQ99-006. This
procurement is being issued unrestricted. The Standard Industrial
Classification is 3826 with a size standard of 500 employees. This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-4. Furnish and
Install to the Appalachian Laboratory for Occupational Safety and
Health, Morgantown, WV the following Contract Line Items (CLINs): (001)
12 Each, TEST CHAMBER PACKAGE WITH FORCE LEVER; (002) 1 Each, INTERFACE
SUPPORT; (003) 1 Each, INTERFACE MODULES AND CABLES; (004) 1 Each,
SOFTWARE AND COMPUTER; and (005) TRAINING. All CLINs shall be quoted in
accordance with the following specifications: SPECIFICATIONS FOR EACH
TEST CHAMBER (quantity 12): -- Standard size for rodents (rats) --
Chambers shall be modular for easy changing of response manipulanda or
stimulus modules -- Stainless steel waste pan -- Stainless steel grid
floor -- House light with 28V bulb (100mA); shall be modular for easy
replacement or movement -- Stimulus light with 28V bulb (100mA); shall
be modular for easy replacement or movement -- White noise generator;
shall be modular for easy replacement or movement -- Clicker/Relay
module to provide an auditory stimulus; shall be modular for easy
replacement or movement -- Pellet dispenser (silent type) for use with
45mg standard rodent pellets -- Trough-type pellet receptacle -- Force
lever equipped with an aluminum paddle measuring 1/4" by 1-7/8"
protruding into the chamber no more than 3/4" (lever also must be able
to be shortened or mounted outside of the chamber), a microswitch to
indicate a lever press, and a 0-600gram load cell and amplifier to
record dynamic force. SOFTWARE SPECIFICATIONS (site licenses if
necessary): -- Windows compatible software to program all experimental
events data collection. -- Ability to program specific force
requirements on the levers and to make reinforcement contingent on
meeting a specified force criterion. -- Software shall be able to
control multiple chambers and/or experiments that run concurrently. --
Ability to record data in real time. -- Ability to record dynamic
force data -- Ability to display cumulative records of responding in
real time for one or more chambers concurrently. -- Ability to save
real time cumulative record data to files for analysis at a later time.
-- Software shall include utilities for easy extraction of data from
saved files. COMPUTER AND INTERFACE SUPPORT SPECIFICATIONS: -Test
chambers are to be independently controlled by one or more computers.
Any and all interface support, including computers (one or more for
experimental control and one for data display), power supplies, relay
rack, cables, and computer interface cards should be supplied to meet
the hardware and software requirements listed above. DELIVERY OF ALL
ITEMS SHALL BE 10-15 WEEKS AFTER RECEIPT OF ORDER. The Government will
evaluate based on F.O.B. Destination (Morgantown, West Virginia).
Shipping and Handling costs shall be included in the price or
identified as a separate line item. The requirements at FAR 52.214.21
"Descriptive Literature" apply. Information provided must demonstrate
the unit's ability to meet the specifications stated above or your
offer may be rejected. The following FAR provisions apply to this
solicitation: FAR 52.212-1, Instructions to Offerors -- Commercial
Items; 52.212-4, Contract Terms and Conditions -- Commercial Items and
52.212-5 Contract Terms and Conditions Required to Implement Statutes
or Executive Orders -- Commercial Items. Additionally, the following
FAR clauses cited in Clause 52.212-5 are also applicable to
thisacquisition as follows: 52.222.26 "Equal Opportunity"; 52.222-35
"Affirmative Action for Special Disabled and Vietnam Era Veterans";
52.222-36 "Affirmative Action for Handicapped Workers"; 52.222-37
"Employment Reports on Special Disabled Veterans and Veterans of
Vietnam Era"; 52.225-3 "Buy American Act -- Supplies"; 52.225-18
"European Community Sanctions for End Products." Quotes are due
December 21, 1998, 2:00 p.m. local prevailing time. Fascimile
submissions are not authorized. Submit the following to DHHS, PHS, CDC,
NIOSH, ALOSH, Attn: Brenda Goodwin, 1095 Willowdale Road, Morgantown,
WV 26505: (1) original and two copies of a completed SF 1449, signed by
an individual authorized to bind the organization; (2) Detailed Pricing
Quotation for each line item; (3) Completed Representations and
Certifications -- Commercial Items, FAR 52,212-3; (4) descriptive
literature and other information demonstrating compliance with the
specification, for evaluation purposes; and (5) Acknowledgment of
solicitationamendments, if any. The Government anticipates one award to
the offeror whose quote offers the best value to the Government. Offers
that fail to furnish the required information, or reject the terms and
conditions of the solicitation may be excluded from consideration.
