Blanket Purchase Agreement for pharmacist and pharmacy technician
services. Performance shall include all personnel, equipment,
facilities, supplies, permits, services, materials, transportation, and
supervision necessary to provide these services in accordance with all
applicable laws and regulations, standards established by the Joint
Commission on Accreditation of Health Care Organizations, standards
established by the American Society of Hospital Pharmacist, and
professional and technical standards of the health care industry in
general. Pharmacist duties shall include the following activities:
daily interactions with prescribers, pharmacist interventions, counter
detailing, formulary reviews, target drug programs, drug utilization
and cost effectiveness studies, patient education, prevention of
adverse outcomes, monitoring for poly-pharmacy patients, and monitoring
for controlled substance misuse. Pharmacy technician duties shall
include the following activities: review prescriptions, fills
prescriptions, repackage bulk pharmaceuticals, monitor/order stock
supplies, input prescription data, replenish medications in automated
drug dispenser, and other related tasks. Pharmacist must posses the
following qualifications: graduate of an approved college of pharmacy,
possess a valid license to practice in the state of Florida, and be
board eligible or board certified. Pharmacy technician must possess the
following qualifications: training and/or experience equivalent to that
required to possess a Class A pharmacy technician's license in the
state of Florida, have a minimum of one year experience within the last
three years in hospital pharmacy for technicians assigned to inpatient
pharmacy positions, have a minimum of one year experience within the
last three years in retail or hospital pharmacy for technicians
assigned to outpatient pharmacy positions, and speak, understand, read,
and write English fluently. The Standard Industrial Classification Code
is 8049; small business size standard is $5.0M average annual receipts
over past three years. The proposed request for quotation shall be
