synopsis/solicitation for commercial service prepared in accordance
with the format in FAR Subpart 12.6, as supplemented, with the
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. Solicitation number RFQ
667-04-99; award will be made using the simplified acquisition
procedures. This solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular
97-09. This acquisition is unrestricted. The standard industrial
classification (SIC) 7361. REQUIREMENT: Contractor to provide two (2)
Anesthesia/CRNA, all inclusive rate, to perform services, approximately
40 hours per week for approximately nine months. Contractor shall
provide all liability insurance, services, lodging, transportation,
communications and associated expenses for the contract period.
Individual performing under this contract shall provide the necessary
documentation for privileging purposes at this VAMC. Work shall
commence on January 4, 1999 through September 30, 1999 Surgical
procedures and services include but are not limited to: Lower Back,
upper back, bladder, renal, lungs, thoracotomy, cataract, cystoscopy,
prostate, hernia, bronchoscopy, nasal, arthroscopy, total hip,
amputation, arterial, laparotomy, abdomen and ureteral. No open heart
or transplants. TERM OF THE CONTRACT: Services shall be provided at the
Overton Brooks VAMC, 510 East Stoner Avenue, Shreveport, LA 71101,
Monday through Friday from 7:30 am to 4:00 pm, except for federal
holidays. National Holidays: The ten (10) holidays observed by the
Federal Government, i.e., New Years Day, President's Day, Martin Luther
King Jr.'s Birthday, Memorial Day, Independence Day, Labor Day,
Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. The
contract resulting from this solicitation shall be for a period of 9
months with the option to extend for up to 12 one month periods.
Additionally we shall have the option to increase the quantity with 10
day written notice, FAR 52.217-6. PAYMENTS: Sums due, the Contractor
will be paid monthly, in arrears, upon receipt of a properly prepared
invoice FAR 52.232-25. The Contractor shall submit all invoices to the
following address: Chief Fiscal Service (04) Overton Brooks VA Medical
Center 510 East Stoner Ave. Shreveport, LA 71101. QUALIFICATIONS:
Personnel under this contract shall be licensed in a State, Territory,
or Commonwealth of the United States or the District of Columbia and
Board Certified. The qualifications of such personnel shall also be
subject to review by the VAMC Chief of Staff and approval by the VAMC
Director. PERSONNEL POLICY: The contractor shall be responsible for
providing: -- Health examinations. -- Worker's compensation. --
Professional liability insurance. -- Income tax withholdings. -- Social
security payments. The parties agree that the contractor shall not be
considered a VAMC employee for any purpose. CONTRACT PERFORMANCE: a.
The contractor will be monitored through a variety of mechanisms,
including but not limited to: -- Clinic appointment lists showing
patients seen. -- sign-in/out log. -- Chart documentation of patient
diagnosis, treatment and or follow-up procedure. -- QA monitors
designed to asses the quality of care provided in the clinic. -- Log
books to document Surgical procedures. b. The Administrative Officer
for Surgery (112) will be responsible for monitoring the quality of
services provided under this contract. Estimated Hours 2880 _________

acquisition. There is not an addenda to these provisions. The clauses

by reference, with the same force and effect as if they were given in

solicitation, best meets the minimum needs of the Government and has
