commericial items prepared in accordance with the format in subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
request for quote number is RFQ 520-10-99. This solicitation document
and incorportated provisions and clauses are those in effect through
Federal acquisition Circular 97-06. The following FAR provisions apply
to this solicitation: 52.212-1 Instructions to Offerors -- Commercial
Items. All offerors are to include a completed copy of the provision
52.212-3 Offeror Representations and Certifications -- Commercial
Items. The following FAR clauses apply to this acquisition: 52.212-4
Contract Terms and Conditions -- Commercial Items; 52.212-5 Contract
Terms and Conditions Required to implement Statutes or Executive Orders
-- Commercial Items, along with the following additional FAR clauses,
cited in Clause 52.212-5: 52.222-26 Equal Opportunity (E.O. 11246),
52.222-35 Affirmative Action for Disabled Veterans and Veterans of the
Vietnam Era (38 U.S.C. 4212), 52.222-36 Affirmative Action for Workers
with Diabilities (29 U.S.C. 793), 52.222-37 Employment Reports on

52.225-3 Buy American Act Supplies (41 U.S.C. 10). The following FAR
cluase is added by addemdum: 52.214-21 Descriptive Literature. The
following VAAR clauses and provisions are added by addendum: 852.211-70
Requirements for Operating and Maintenance Manuals, 852.219-70
Veteran-Owned Small Business, 852.270-4 Commercial Advertising. The VA
Medical Center, Biloxi MS will be requiring at a minimum the following
requirements for two (2) small piece folder systems: Furnish and
install two (2) each microprocessor-controlled small piece folders
capable of folding pajama tops, pajama bottoms, robes, terry bath
towels, bed pads, gowns, fitted sheets, etc. Folder shall be located
near the folding tables so they may be used in tandem. Folders must
meet the following minimum requirements: 1) Machine is capable of being
programmed for fifty (50)different items and folds. 2) Machine is
capable of being programmed by keypad for half folds, quarter folds,
and one third folds. 3) Machine is capable of folding terry towels,
hand towels, bed pads, pajama tops, pajama bottoms, bibs, adult, and
fitted sheets. 4) Machine is capable of stacking in increments of 5's,
10's, 20's, and 25's. 5) Machine has quality grading controls for
stained torn items. 6) Electrical requirements: 208/480 volts, 3 phase,
60 hertz. 7) Four (4) each lockable castors required. 8) Single
operator with return-to-feed converyor, folded items are stacked on
delivery conveyor with a choice of a discharge direction. 9) LED piece
counter required. 10) French fold items up to 36" x 72" (915mm x
1,830mm). 11) Linen items up to 26" (660mm) wide may also be half
folded. 12) Primary fold may be passed for direct crossfolding of items
such as bath mats or surgical pants. 13) Mircroprocessor with message
screen for set-up, opertaing, testing, and diagnostic functions. 14)
Powered width adjustment. 15) Stain/tear reject button discharges items
separately. 16) Auto jam shutdown with audible jam alarm and job
reverse features. 17) Air pressure regulators for each folding section
assure peak performance. 18) Folds items up to 96" (2,440mm) in
length. 19) Size requirements: Equipment to to exceed 150" x 100" x
100" (3,810mm x 2,540mm x 2,540mm). Additional requirements: 1)
Training: The successful offeror shall provide on-site orientation and
training to using personnel in operation and care of the equipment
furnished. This training shall include actual demonstration and
operation of the equipment, preventative maintenance, and adjustments
or other actions which may be undertaken by operating personnel in the
event of failure of the equipment, provided that each adjustment or
action will in no way jeopardize the equipment warranty. This training
shall be given by qualified representatives of the manufacturers of
the equipment furnished for a period of three (3) workdays (exclusive
of travel time). 2) The contractor shall provide a list of preventative
maintenance requirements. 3) The successful offeror shall provide four
(4) sets of bound service data manuals for each piece of equipment
that includes installation instructions, electrical and plumbing
schematics/diagrams, manufacturer's spare parts listings and
preventative maintenance procedures. 5) Required production standards
for each item (minimum folded items per hour) are as follows: Pajama
top -- 545/hr, pajama trousers -- 600/hr, surgical gowns -- 300/hr,
isolation gowns -- 300/hr, patient gowns -- 600/hr, scrub shirts --
1,000/hr, scrub pants -- 750/hr, bath robes -- 1,200/hr, underpads --
500/hr, Bibs, adult -- 1,000/hr, mattress pads -- 300/hr, fitted sheets
-- 300/hr. The following government-owned equipment in "AS IS -- WHERE
IS"condition is available for trade-in: Compact towel and gown folder,
two (2) each, size 100 x 50 x 63., Manufacturer: Jensen, Model:
Wishbone, Serial numbers: 586-B1-1042 & 586-B1-1041, Acquisition date:
July 1985. Delivery must be within 30-45 days after award of contract.
The units must meet the standards above at a minimum. Offerors must
submit brand name, model names, and warranty for each unit being
offered. Note: Descriptive Literature: Bidders are cautioned that the
reponsiveness of their bid depends on the completeness and suffciency
of descriptive material submitted with the bid. Descriptive literature
must identify the manufacturer, make, and model of the item offered.
Bidders must highlight descriptive literature to show the item meets
the minimum requirements. Year 2000 Compliance: The Offeror shall
submit certification to this VA Medical Center that the items being
offered are Year 2000 Compliant. Prices must be FOB destination,
Biloxi, MS. Award will be based on price. The VA intends to award a
single contract and reserves the right to make multiple awards, if in
the Government's best interest. Offers are due not later than close of
business, 4:30pm CST, January 14, 1999. Offers in original and one (1)
