The Portland VA Medical Center (PVAMC) located at 3710 SW U.S. Veterans Hospital Road, Portland, Oregon, requires the use of space, administrative support, and ancillary services, (e.g., laboratory, x-ray, and EKG services), to provide primary care to PVAMC beneficiaries residing in nine counties of Central Oregon: Crook, Klamath, Deschutes, Lake, Grant, Wasco, Harney, Wheeler, Jefferson. PVAMC providers will be responsible for managing the PVAMC beneficiaries' primary medical care in the occupied space. To be considered for award, the offeror must demonstrate: (1) existed as a business entity providing similar services for a minimum of two years as of the date of proposal submission; (2) provide certification that all services (including those to be provided by subcontract arrangements) are properly licensed by the appropriate governing body; (3) be located within a 15 minute drive of Bend, Oregon. Evaluation criteria is described in the Solicitation. It is expected that the contract will be awarded by the end of February 1999 with services to begin within 30 days from contract award. A commercial item request for proposal will be issued on or about December 17, 1998.
