purposes for Minority, Women-Owned and Disadvantaged Business
Enterprises: The Department of Transportation (DOT), Office of Small
and Disadvantaged Business Utilization (OSDBU), has a program to assist
minority, women-owned and disadvantaged business enterprises to acquire
short-term working capital assistance for transportation-related
contracts. Loans are available under the DOT Short Term Lending Program
(STLP) at prime interest rates to provide accounts receivable
financing. The maximum line of credit is $500,000. For furhter

contracting official listed above. SCOPE OF SERVICES HEALTH CLUB
MEMBERSHIP CLEVELAND, OH 1. Purpose: The purpose of this membership is
to provide a source where Coast Guard Employees can exercise on a
daily basis. 2. Overview: a. The Coast Guard believes that physical
fitness is paramount in maintaining an operationally ready military
service. The Coast Guard encourages its military and civilian employees
to participate in physical fitness activities to the maximum extent
allowed by operational commitments. b. In order to encourage and
support Coast Guard Military and Civilian employees assigned to the
Greater Cleveland Area, a "Corporate" style health club membership is
sought. This membership at a convenient location will provide Coast
Guard employees with a place to exercise. 3. Requirements: a. Health
Club must be conveniently located within a 1.5 mile radius from the
Anthony J. Celebreeze Federal Building located at 1240 East Ninth
Street, Cleveland, OH. Most employees exercise during business hours
and a close location is necessary to allow employee's time to leave
their job site, arrive at the club, work out, and return to the job
site in a one and a half hour time frame. b. Coast Guard employees will
consist of Active Duty Military Personnel, Full Time Civilian
Employees, and Coast Guard Reserve Personnel. Only Coast Guard
employees assigned to the Greater Cleveland Area are eligible. c. All
services must be available to Coast Guard employees on the same basis
as regular members of the health club. d. We are seeking continued
unlimited usage for 250 personnel. Current daily (Monday through
Friday) usage based on one year membership. e. Access to the club with
a Coast Guard ID. 4. Specific Information Required: a. Health Club (1)
Hours and days of Operation (2) Type of programs, services, equipment
available (see attached enclosure) (3) Type of staff available (4) Does
staff provide instructions on proper use of exercise equipment? (5) A
method of tracking and reporting usage (6) Parking availability b. Cost
(1) Annual cost of corporate style membership (2) Additional
services/amenities offered (3) Coast for guest passes (4) Parking Costs
5. Duration: Club membership for the period 6. Base Period: October 1,
1998 through September 30, 1999 Option 1: October 1, 1999 through
September 30, 2000 Option 2: October 1, 2000 through September 30, 2001
Option 3: October 1, 2001 through September 30, 2002 Option 4: October
1, 2002 through September 30, 2003 all in accordance with this
Statement of Work The following Federal Acquisition Regulations
Provisions and Clauses apply: 52.217-5 Evaluation of Options.
Evaluation of Options (Jul 1990) Except when it is determined in
accordance with FAR 17.206(b) not to be in the Government's best
interests, the Government will evaluate offers for award purposes by
adding the total price for all options to the total price for the basic
requirement. Evaluation of options will not obligate the Government to

52.217-9 Option to Extend the Term of the Contract. Option to Extend
the Term of the Contract (Mar 1989) (a) The Government may extend the
term of this contract by written notice to the Contractor within
[insert in the clause the period of time in which the Contracting
Officer has to exercise the option]; provided, that the Government
shall give the Contractor a preliminary written notice of its intent to
extend at least 60 days before the contract expires. The preliminary
notice does not commit the Government to an extension. (b) If the
Government exercises this option, the extended contract shall be
considered to include this option provision. (c) The total duration of
this contract, including the exercise of any options under this

Availability of Funds (Apr 1984) Funds are not presently available for
this contract. The Government's obligation under this contract is
contingent upon the availability of appropriated funds from which
payment for contract purposes can be made. No legal liability on the
part of the Government for any payment may arise until funds are made
available to the Contracting Officer for this contract and until the
Contractor receives notice of such availability, to be confirmed in
