is a combined synopsis/solicitation for commercial items prepared in
accordance with the format in Subpart 12.6 as supplemented with
additional information included in this notice. This notice shall
constitute the only solicitation and a written solicitation will not be
issued. This Basic Ordering Agreement shall provide initial evaluation
intake and testing services for veterans who establish basic
eligibility under Chapter 31, Title 38, U.S.C. as authorized by a case
manager from the Vocational Rehabilitation and Counseling (VR&C)
Division of the Veterans Administration Regional Office, Cleveland,
Cincinnati and Columbus Ohio. Services include but not limited to
administering, scoring and providing summaries of results for all tests
administered at the request of VA; as well as taking a client history,
evaluating the veterans rehabilitation assets and liabilities; making
recommendations regarding appropriate rehabilitation services and
expected outcomes, and summarizing all information, evaluations and
recommendations in a comprehensive report. The client history will
include, but will not be limited to the following: personal and
familial history; education/training history; military service record;
employment history; current financial circumstances; service connected
and non-service -connected disability conditions; any potential
barriers to successful vocational rehabilitation. Contractor shall
provide names and qualifications of individuals who will perform
services under this contract to the Contract Specialist with submission
of their proposal. Contractor shall report and submit for approval any
proposed changes in personnel providing services under this contract
to the Contract Specialist. All staff changes will be subject to
evaluation criteria specified in the solicitation. Contractor shall
schedule the initial testing and/or intake counseling session within
seven (7) calendar days of the date of authorization by the VA Case
Manager. Contractor's place of business shall be based in Ohio and able
to provide services as needed in the Ohio counties of Franklin,
Guernsey, Washington, Jackson, Ross, Morrow, Logan, Wayne and
contiguous counties at locations designated by a VA Case Manager. VA
Case Manager or designee are authorized to visit the premises of the
contractor during business hours for purposes of evaluation and
auditing which may include inspection of records. Period of performance
shall be from January 19, 1999 through September 30, 1999 with one (1)
year renewal option from October 1, 1999 through September 30, 2000.
Proposals must include pricing for Base Year (1-19-99/9-30-99) and
Option Year 10-1-99/9-30-2000). This procurement is a 100% small
business set aside. SIC for this procurement is 8331. Proposals shall
be evaluated on the following criteria and evidence to support the
evaluation criteria must be submitted with your proposal. (1) Previous
experience and present expertise in vocational rehabilitation
counseling, testing and initial evaluation of disabled veterans. To
include: Number of disabled veterans seen previously for counseling,
testing and initial evaluation; types of services provided; approximate
date (by year) of cases by category of service provided; Agency or
counseling setting in which such services were provided; average number
of sessions by type of services provided; an indication of the length
of time services were provided, by case; relevant other special
information regarding prior services provided should be briefly
mentioned. The offeror should indicate any prior experience as a
federal government contractor and performance history in connection
with such contract(s). Information for this item must be provided for
each counselor in detail. The offeror must also indicate the number of
testing and initial evaluation intakes per week that can be provided
under the terms of this contract by all staff members with the required
credentials and prior experience providing services to disabled
veterans (100 points). (2) A commitment in principle to providing
services to disabled veterans as evidenced by its bylaws, mission
statement, or established statement of goals and procedures. (10
points) (3) An established program, described in writing for providing
disabled veterans the following services: Administering, scoring and
providing summaries of results for all tests administered to vocational
rehabilitation clients; recording the client's history; evaluating the
client's rehabilitation assets and liabilities; making recommendations
regarding appropriate rehabilitation services and expected outcomes.
The contractor must be able to directly furnish the services described
above. (50 points) (4) The use of individualized plans for rendering
service to clients. Documentation might include a detailed description
of and any forms used in recording the client history, evaluating the
veterans rehabilitation assets and liabilities, making recommendations
regarding appropriate rehabilitation services and expected outcomes and
summarizing all information, evaluations and recommendations in a
comprehensive report. (10 points) (5) Demonstrated procedures to
safeguard records to include detailed documentation describing the
system for maintaining the physical security of records and prevention
of disclosure of the records, reports, or other private information of
clients, except with client's informed written consent. (10 points) (6)
Contractor's personnel performing under this contract shall meet the
following minimum education requirements: (a) A doctoral degree in
vocational rehabilitation counseling, counseling, psychology, social
work, or another closely related field from an accredited educational
institution; or (b) A master's degree in psychology, counseling, social
work or another closely related field from an accredited educational
institution and the professional credentials of Certified
Rehabilitation Counselor (CRC) from the Commission on Rehabilitation
Counselor Certification; or Licensed Professional Counselor (LPC),
Licensed Professional Clinical Counselor (LPCC) or Licensed
Professional Social Worker from the State of Ohio Counselor and Social
Work Board. A detailed vitae including a description of any formal
training and education in the areas of taking client's histories,
evaluating veterans rehabilitation assets and liabilities and making
appropriate recommendations for rehabilitation services and expected
outcomes must be submitted with proposal. (100 points) The Department
of Veterans Affairs Medical Center will award Basic Ordering
Agreement(s) in rank order with the first award going to the offeror
that receives the highest number of points during the evaluation
process. The proposal with the lowest price may not necessarily be
awarded. The following provisions apply to this solicitation and may be
requested in writing to the Contract Specialist listed above.
