MIKE NYE, 619/532-8088 This is a combined synopsis/solicitation for
commercial items, prepared in accordance with the format in FAR Subpart
12.6 as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; quotations
are being requested and a written solicitation will not be issued. The
solicitation, number N00259-99-Q-0024, is issued as a Request for
Quotations (RFQ). The solicitation document and incorporated provisions
and clauses are those in effect through FAC 97-09. This solicitation is
100% set-aside for small business. Non-personal services are requested
for four (4) full-time phlebotomist and one (1) part-time
phlebotomist. Services shall be provided in the Laboratory Department
of the Naval Medical Center San Diego for the period 01 Feb 99 through
30 Sep 99. The contractors shall provide, on behalf of the government,
phlebotomy services for treatment of active duty military personnel,
their dependents, eligible Navy civilian employees and other eligible
beneficiaries. The contractors shall be on duty at various blood draw
sites located at the Naval Medical Center San Diego for 40 hours per
week. The full-time phlebotomists shall normally provide services for
an 8.5 hour period (to include an uncompensated .5 hour for lunch)
between the hours of 0500 and 2300; part-time phlebotomists shall
normally provide services for a 4 hour period with no lunch break
provided. Specific hours of duty to be arranged as needed. The
contractors shall: provide training and/or direction as applicable to
supporting Government employees (i.e. hospital corpsmen, technicians,
students) assigned to them during the performance of clinical
procedures; perform limited administrative duties; participate in
weekly meetings; attend annual renewal of Annual Training Requirements
(family advocacy, disaster training, Sexual Harassment and other
courses as directed); provide services in accordance with all State,
County, Department of Defense, Navy and NMC San Diego guidelines and
reporting requirements; and participate in the implementation of the
Hospital's Family Advocacy Program as directed. Clinical Duties:
Routine workload is scheduld by the treatment facility. Primary
workload is related to the provision of medical care by staff health
care providers through written or verbal orders. Secondary workload is
the result of consultation requests submitted tot he Laboratory by
government staff practitioners. The contractors shall have full
responsibility for maintenance and delivery of comprehensive applicable
phlebotomy services within the personnel and equipment capabilities of
the facility, and for the quality and timeliness of record preparation
and reports indicated to document care and procedures given. The
contractors shall perform a full range of phlebotomy procedures, on
site, using Government furnished facilities, equipment and supplies.
Clinical Functions: The contractors shall: conduct patient
in-processing prior to specimen collection, including review of
paperwork accompanying patients and cordial guidance and assistance;
perform proper venipuncture, finger stick and heel stick procedures
using appropriate techniques; perform set-up and completion of special
test-related phlebotomy procedures utilizing appropriate techniques;
adhere to laboratory safety protocols; perform phlebotomy-related
administrative and clerical procedures of the Front Desk Section of the
Lab; fully document all samples obtained per the laboratory's standard
operating procedures; interact with other employees and lab management
to provide efficient specimen processing service to patients; perform
centrifugation procedures as instructed by lab management. Personnel
Qualifications-the following are required: (1) Have one of the
following: either a degree or certificate of graduation from an
approved/accredited phlebotomy training program OR 6 months experience
as a phlebotomist within the past 24 months OR a national or state
recognized certification as a phlebotomist; (2) Provide letters of
recommendation from three lab supervisors, clinical teachers or
physicians attesting to the clinical skills. Reference letters must
include name, title, phone number, date of reference, address and
signature of the individual providing reference. Letters of reference
must have been written within the preceding 5 years; (3) Current
certification in American Heart Association Basic Life Support (BLS)
for Healthcare Providers; American Heart Association Healthcare
Provider Course; or American Red Cross CPR (Cardio Pulmonary
Resuscitation) for the Professional Rescuer; or equivalent; (4)
Represent an acceptable malpractice risk to the Navy. Submit a signed,
detailed statement of prior or pending malpractice claims and an
explanation of disposition of each; (5) U.S. employment eligibility. No
alien shall be allowed to perform under this contract in violation of
the Immigation Laws of the United States; (6) A written Personal
Qualifications Form and supporting documentation which includes an
acknowledgement of the requirement and a current resume or Curriculum
Vitae; (7) A signed and dated Health Care Worker Certificate of
Availability for each individual healthcare worker submitted with their
proposal. Certification shall be signed and dated after solicitation
issue date. Regulatory Compliance Requirements: The contractors shall
obtain, at contractor expense, a statement from the health care
worker's physician or a report of a physical examination within 60 days
prior to contract start indicating that the healthcare worker is free
from mental or physical impairments which would restrict the health
care worker from providing the services described herein. Health care
workers hired after contract start shall be required to provide a
physical exam statement or report obtained within 60 days prior to

EVALUATION-COMMERCIAL ITEMS (Oct 95) is incorporated by reference with
the following addenda: in paragraph (a) the government will evaluate
proposals based on technical capability and cost to the government. All
other factors when combined are significantly more important when
