This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. The solicitation number is: F38601-99-RA005 is a Request for Proposal. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-08. The requirement is 100% set-aside for Small Business. The Standard Industrial Classification Code is: 1781 and the small business size standard is: $7M. 3. Provision at FAR 52.212-1, Instructions of Offerors Commercial apply. 4. A completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items will be submitted with offer. 5. Clause at FAR 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. 6. Clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statues or Executive Orders Commercial Items, applies to this acquisition. 7. REQUIRED SERVICE: Requirement exists for repair of Water Well #3, Shaw AFB SC. This requirement is being solicited under FAR Part 12. The pump is currently gravel locked in the well. The inner casing is 10-inch diameter with a 20-inch outer casing. The depth of the well is 325 feet with a 75 HP motor. Submit offers to provide a complete and usable well connected to be fully operational. The desired goal to pump 600 gallons per minute. Well must be chlorinated in accordance with State Health Department procedures and bacteriological tested until satisfactory results are obtained. Well must be vibration tested on completed assembly and adjustments made if vibration exceeds the allowable for this type of installation. Should a site visit be required, immediately contact the contract negotiator listed below to request such a visit. Estimated completion time to repair well is 120 days after receipt of order. Government anticipates award without discussion but reserves the right to enter into discussions with any bidder/potential bidder at any time. A firm-fixed price contract is contemplated. 8. SUBMISSION OF OFFERS: Submit offers to: 20th Contracting Squadron, ATTN: Contract Negotiator: Captain Ray Noble, 305 Blue Jay Street, Shaw AFB SC no later than: 26 Jan 1999, 1200 noon. Proposals will include pricing information and description of approach to performing the work. Offerors will also complete certifications required by FAR 52.212-3. Offerors will also submit at least two companies as references. Past performance references should be for work similar to this requirement. Furnish the company name, address, point-of-contact to include telephone number, and contract number. The past performance information shall be furnished as soon as possible, but not later than the date set for receipt of offers. 9. EVALUATION CRITERIA: Evaluation of offers will be an integrated assessment of: Price, approach, and past performance. Submit price and description of your approach to completing the project to include any drawings or descriptive literature. 10.
