Transamerica Occidental Life Insurance Company (TOLIC) which currently is under contract with HCFA for Medicare Part B claims processing is seeking sources for the printing and distribution of the 1999 Medicare Billing Manual. Delivery will be to Los Angeles office with distribution mailed by vendor. I) TECHNICAL REQUIREMENTS: 1) Booklet will be 11" x 17" folds to 8 " x 11", 2) Cover will be card stock 3) Two (2) ink colors -red (PMS 186U), and blue -(PMS 280U), 4) Paper -- white 60#, 5) Binding -- Standard 3 hole drill, shrink wrap for mailing, 6) Quantity is approximately 27,000 7) final (8 " x 11") page count of approximately 200 pages. II) OTHER REQUIREMENTS: 1) Mailing -- Bulk-rate Postage permit, 2) A 3480 cartridge or 3.5" diskette will be provided containing mailing list. (Please specify preference.) Vendor is responsible to generate labels or inkjet spray addresses on mailing, 3) Postage -- bulk rate. Vendor is responsible for any prepayment. 4) Artwork will be available on 600 DPI laser print copy or on 3.5 diskette (Specify preference, 5) Capability to incorporate late text changes in one day, 6) Project must be completed within 15 days after receipt of artwork. Interested parties must respond no later than February 8, 1999 to obtain a copy of the detailed specifications and proposal criteria. Quotations must be submitted no later than February 22, 1999. Responses will be reviewed and evaluated 70% on cost and 30% on meeting the specific requests as detailed in specifications and proposal criteria.
