The work includes the addition of a 3-story, above grade clinic building and a 3-level, 250-car parking structure, connected to the existing hospital structure through a new 3-level quarterdeck/lobby structure. The work also includes the remodel of portions of the existing hospital on levels 1, 2, 3, 6, sublevels, and incidental related work. The project is phased construction with the existing hospital in full operation during construction. The Standard Industrial Classification Code is 1542. The related small business size standard is $17 million. The estimated cost for this procurement is more than $10 million. The project has funding profile limitation of $24 million. This project will be procured using source selection procedures; therefore, techical, socio-economic program, and price proposals will be required. The technical evaluation factors are: (1) Experience and Past Performance; (2) Scheduling; and (3) Key Personnel & Resources. The socio-economic program factor will be Subcontracting Experience, Achievements, and Goals. The Government intends to issue this solicitation through the use of CD-ROM and the Internet. Plans and specifications will NOT be provided in a paper (hard copy) format. Plans and specifications will be on CD and available on or about February 22, 1999. There will be a NON-REFUNDABLE charge of $64.00 for a set of CDs. CDs may be obtained by mailing a request, with payment, to EFA NW, ATTN: Plan Issue, 19917 -- 7th Avenue NE, Poulsbo, WA 98370-7570, or via FAX request with credit card number and expiration date to (360)396-0998. Telephone requests will not be honored. All requests for plans and specifications MUST include: (1) complete solicitation number and project title; (2) name, address, and phone number of requesting company; (3) number of CD sets requested; and (4) payment (if using VIAS, Mastercard, or Discover Card include account number and expiration date; if using check or money order make payable to SUPERINTENDENT OF DOCUMENTS; cash will not be accepted). CDs will not be issued until proper payment has been received. CDs will be issued via First Class Mail. NO PICKUPS are authorized. After the issue date, requests will be filled only on Friday of each week (requests must be received by 4:00 PM Tuesday to be issued on the following Friday).
