for commercial items prepared in accordance with the Federal
Acquisition Regulation (FAR) sub part 12.6., as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation for proposals; a written solicitation
WILL NOT be issued. Solicitation number RFQ-797-FDF3-99-0009 is issued
as a Request for Quotation The solicitation document and incorporated
provisions and clauses are those in effect through FAC 90-46. The SIC
code is 3841 and the small business size standard is 500. employees.
The contractor shall provide the following Brand Name or Equal items:
Quantity of 2, Haag-Streit 900 Slit Lamp, Table Model; Quantity of 2,
Haag Streit 50/50 Selectable Beam Splitter for BQ Observer; Quantity of
2, Haag-Streit Cold Light Power Source for Background 2 Illumination
with 72 Cable and Mirror with Holder; Quantity of 2, Sony Video
Package: DXC-107A Color 2 CCD Camera, CMA-D2 AC Adapter, and CCMC-12P05
Cable; Quantity of 2, Sony UP-2100 Color Video Printer; Quantity of 2
Users Manuals. The contractor shall provide delivery: to Defense
Distribution Depot West, Bldg 16-b-1 Medex, South Chrisman Road, Tracy,
CA 95376-5000. Delivery, FOB Depot, shall occur on or before March,
1999. Responsible offerors shall provide the information delineated in
FAR clause 52.212-1, Instructions to Offerors-Commercial Items and a
completed copy of provision 52.212-3, Offeror Representations and
Certification -- Commercial Items. Offerors shall also provide
Commercial and Government Entity (CAGE) Code, Contractor Establishment
Code (DUNS), and Tax Identification Number. Clause 52.212-2,
Evaluation -- Commerical Items, applies to this acquisition. The
Government intends to evaluate offers and award a contract without
discussions. Award will be made to the responsible offeror whose offer
is most advantageous to the Government based on the following factors:
(1) Technical Capability, (2) Past Performance, and (3) Price.
Technical capability shall include, but is not limited to compatibility
with the aforementioned existing items and timeliness of performance
(best delivery date). Past performance shall include, but is not
limited to experience with similar contracts with the Government of
similar magnitude and acceptable performance. Offerors shall provide
technical information and a minimum of three past performance
references, preferably government references. Technical and past
performance when combined are more important than price. The resulting
award will incorporate the requirements of the clause at FAR 52.212-4,
Contract Terms and Conditions -- Commercial Items. Clause 52.212-5,
Contract Terms and Conditions Required to Implement Statutes or
Executive Orders -- Commercial Items, applies to this acquisition.
