This is a combined synopsis/solicitation for commercial items or services prepared in accordance with format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; and a written solicitation will not be issued. This solicitation is issued Request for Quotation (RFQ No. NIA-99-JCC0006). The solicitation clause and provisions are those in effect through Federal Acquisition Circular 97-06. The Standard Industrial Classification (SIC) Code for this acquisition is 3575 and will be utilized for this 100% Small Business Set-Aside. Delivery shall be FOB Destination. NIH/NIA Gerontology Research Center(GRC) intends to purchase one (1) Laboratory Information System. This system is required in order to ensure compliance with CLIA regulations for certification of the newly established Clinical Core Laboratory at GRC. The system configuration should include the following: software for two(2) full main lab workstations, five(5) interfaced main lab instruments, three(3) modified workstations outside lab including phlebotomy/order entry/results retrieval and server and user licenses; hardware to include four(4) bar code label printers, two(2) bar code scanners, expansion cards, cable, hubs, connectors, and fax modem. Installation, training and one(1) year on-line telephone support is to be provided. The following factors will be used to evaluate offers in descending order of importance: 1) technical capability of item offered to meet the government's requirements, 2) price, 3) past performance.
