Treatment will consist of exercise therapy and individualized patient education. The contractor shall provide an appropriately supervised cardiac rehabilitation program for approximately 50 patients per year. These patients will have various forms of heart disease and may be status post myocardial infarction, coronary artery bypass grafting, or PTCA. The exercise/rehab program shall consist of up to 36 sessions. These session shall consist of supervised, progressive exercise and continued patient education. Patients will be constantly monitored while in this phase of the program. Upon completion of this phase, a home program shall be developed for the patient and demonstrated for the patient prior to his/her discharge. An individual with a minimum of 1 year experience shall perform the work. This person shall be ACLS certified. The work may be performed on the contractor's premises or at VAMC Manchester, the equipment provided in the cardiac exercise area. A separate offer shall be submitted for each locations.
