two Special Projects. Project A involves upgrading nine underground
storage tanks and includes cleaning, inspecting coating of tank
interiors, providing high level alarms, and incidental related work.
Project B involves removal of existing plug valves, wooden valve pit
covers, and portions of concrete valve pit walls; provisions of new
plug valves, pit ladders, and aluminum valve pit frames, covers and
doors; concrete patching; and incidental related work. TECHNICAL
EVALUATION FACTORS: The following evaluation factors and subfactors
shall be used to evaluate the technical proposals. The major factors
are listed in order of importance. Factor "A" is more important than
Factor "B". The subfactors within the major factors are of equal
importance. a. FACTOR A. Past Performance/Qualifications/Experience:
The following information shall be submitted for evaluation to
establish satisfactory past performance and qualifications/experience:
(1) For Project A: R5-95, Upgrade Bulk Fuel Storage Tanks: Contractor
must have successfully completed projects that require all of the
following: (a) Past performance of successful completion of cleaning,
inspection, and interior painting of underground fuel tanks in the past
five years. (b) Experience in Tank modification work including welding
done in accordance with API 650 and API 653. (c) Successful
application of modified PTFE-pigmented fluoropolyurethane coating
system on the interior of fuel oil storage tank under conditions
similar to this projects specifications. (d) Demonstrated effectiveness
of Quality Control Program and Accident Prevention Plan for underground
fuel oil tank work. (2) For Project B: R7-95, Upgrade Bulk Tank Farm
Valve Pits: Contractor must have successfully completed projects that
require all of the following: (a) Past performance of successful
completion of fuel piping modification including replacement of valves
in the past five years. (b) Demonstrated effectiveness of Quality
Control Program and Accident Prevention Planfor fuel piping repair
work, including welding in POL areas. (3) Offeror shall submit the
following information for evaluation: (a) List of applicable projects
(Government and/or Private) by contract number, title, description
(include specific work performed as described in paragraphs 2.a.(1) and
(2) above), award amount, completion amount, award date, completion
date, point of contact name, phone number/fax number, and any other
information regarding problems encountered and how resolved. (b) Copy
of Experience Modification Worksheet (obtained from insurance company).
(c) Customers' satisfaction of Contractor's work on similar projects
demonstrated by letters of appreciation, commendations, and past
performance evaluations. Note: Factor A, Past Performance history that
relates to projects similar to this project will receive more
favorable consideration than past history that are not related to this
project. Therefore, the lack of performance history would not
disqualify an offeror, but that, among other things, the offeror could
be considered less favorably than an offeror with a favorable
performance history. b. FACTOR B. Key Personnel: Contractor's
employee(s), or subcontractor(s), will be evaluated for qualifications
and experience in being assigned to this project for cleaning,
repairing, and inspecting underground fuel tanks; painting of tank
interior for underground tanks, inspection of tank coating; and tank
modification work. Submit personnel employment history, education,
qualifications and resume to include description of projects within the
last five years. Projects similar to Projects A and B will receive more
favorable consideration. Provide information for the following
personnel: (1) Individual(s) who will perform the following tank work:
cleaning, inspection, interior coating, and modification work. (2)
Project Superintendent (3) Quality Control Manager (4) Safety Manager.
The estimated cost is $500,000.-$1,000,000. This Request for Proposal
(RFP) is unrestricted. The Standard Industry Classification (SIC) code
is 1629 and the annual size is $17.0 M. THE RFP WILL BE AVAILABLE ON OR
ABOUT 16 FEBRUARY 1999 AT A NON- REFUNDABLE CHARGE OF $70.04. SEND YOUR
