A MILITARILY CRITICAL TECHNICAL DATA AGREEMENT, DD FORM 2345, is required. The form and all applicable instructions can be obtained at the following web site: www.dlsc.dla.mil/ccal. Contracators must have the form filled out and submitted to DLSC and be certified by that organization. A solicitation package can not be forwarded to a contractor who has not been certified. A copy of the Certified DD Form 2345 is required to be submitted with any request for the solicitation package. Any contractor that requests a bid package without a certified DD Form 2345, will have their request returned. Please be sure to put bid number on the form.
