Each RFQ is for one of seven custom packs. The seven packs are entitled: Eye Plastic Pack, Eye Cataract Pack, Total Hip Pack, Hand Pack, Knee Arthroscopy Pack, Shoulder Pack and Total Knee Pack. The packs each contain approximately 20 to 30 different items to include various sponges, drapes, surgical blades, syringes, needles, towels, and other components. Estimated usage is from 100 to 728 packs per year, with delivery through the prime vendor distributor. Contract period is from date of award through September 30, 1999, with two 12-month renewal options. A nonsterile bid sample is required at time of bid opening for each RFQ. Request bid documents in writing or by fax to 612-725-2072. All seven solicitations will be mailed to each respondent. Requests for bid documents will not be honored after February 25, 1999.
