The Veterans Affairs Medical & Regional Office Center (VAMROC) located at 2925 DeBarr Road, Anchorage, AK 99508, has a need for Dental Laboratory Services and Constructed Product Services for the VAMROC's Dental Service. Contractors will need to provide, parts, labor, materials, supervision, equipment, pick-up and delivery services upon request by the Contracting Officer or designee. Contract period shall be April 1, 1999, through March 30, 2000, as the Base Year with four(4) one(1) year option to extend. If all options are exercised the contract period will be through March 30, 2004. All pricing as proposed is FOB destination and no additional fees for delivery will be allowed except in emergent cases. Bids will only be accepted from contractors who are Joint Commission on Accreditation of Healthcare Organizations (JCAHO) or equivalent approved. A copy of the contractors certification must accompany the offer. Failure to provide the certification or equivalent will make the offer nonresponsive. Bids will be considered only from bidders who are regularly established in the business called for and who are financially responsible and have the necessary equipment and personnel to furnish service in the volume required for all items under this contract. Successful bidder shall also meet all requirements of Federal, State or City codes. Bids will be considered only from bidders who are currently accredited by the Joint Commission on Accreditation of Healthcare Organization (JCAHO) or equivalent and/or are currently seeking accreditation.
