65 -- ELECTROPHYSIOLOGY MAPPING SYSTEM SOL RFQ 580-60-99 DUE 030899 POC Contracting Officer, Cathy Edwards, (713) 794-7423 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as request for quotation (RFQ) 580-60-99. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-10. The applicable standard industrial classification code is 5047 with a size standard of 100 employees. Pursuant to FAR 6.302-1, this is a sole source procurement from CordisWebster (Biosense) in Diamond Bar, CA; this system must be compatible with an existing Prucka electrophysiology (EP) system. Offeror should provide a quotation for the following line items (quantity is one each unless shown otherwise): (1) M-5385-37 Carto System with EOD; (2) M-5382-06 SG 02 workstation; (3) M-5404-04 15' power strip; (4) M-5404-03 6' power strip; (5) M-5401-02 Isolation transformer 1.5 KVA; (6) M-5401-01 Isolation transformer .5 KVA; (7) M-5453-01 Anthro cart; (8) M-5389-04 Epson Printer; (6) Operating and service manuals for Carto System -- 2 each; (7) Complete turn-key installation of Carto System -- 1 job; (8) Maintenance training for full service repair of the Carto System to include tuition, lodging and travel -- 1 job; (9) User training -- 1 job. Delivery should be made FOB destination to the VA Medical Center in Houston, TX within 30 days of receipt of order. The provisions of clause 52.212-1 Instructions to Offerors -- Commercial, applies to this acquisition. To be acceptable the proposed system must be FDA approved, compatible with the existing Prucka EP system, and Y2K compliant. Specifics on Year 2000 Warranty -- Commercial Supply Items requirements may be obtained from the Contracting Officer.
