STATEMENT OF WORK =20 C.1 GENERAL. The contractor shall provide
counseling services for the = Navy=20 Family Service Center (NFSC)
located in Saratoga Springs, NY. The = following=20 are the
requirements that shall be met in providing the required =
counseling=20 services to Navy and other military families. The
Contractor must = provide=20 screening, evaluation, information, and
referral, initial and brief = term=20 counseling, individual, couple,
and family counseling. The contractor=20 employees shall conduct
telephone, walk-in, and referral services by = receiving=20 referrals
from FSC Counseling staff. Services shall include, = non-medical=20
individual, couple, and family relationship counseling for all ages of
= military=20 and their family members who cannot be served through
other FSC = specialized=20 Programs and Services. Contractor employees
shall participate in weekly = clinical case consultations, meet with
the Clinical Supervisor for = Clinical=20 Quality Control Auditing
Review, clinical supervision on work = performance, FSC=20
administrative staff meetings, and training sessions held for FSC =
Counseling=20 staff. The Contractor employees shall demonstrate
evidence of = providing brief=20 term counseling. Usually brief term
counseling will be no more than = ten (10)=20 sessions. If more than
ten sessions is desired/required, this must be=20 discussed with and
approved bythe Family Service Center Director. = Generally=20 accepted
brief term counseling strategies such as structural, = behavioral,
problem focused, brief therapy approaches only can be used to
provide=20 counseling to FSC clients. Long term counseling,
introspective, = psychoanalytic=20 approaches to counseling are not
authorized. Counseling is authorized = for=20 work/life relationship
issues only. Contractor employees shall assist = as a=20 primary
counseling source for walk in and telephone counseling cases to =
the=20 FSC. Counseling sessions are to be established for 50-55 minute
= periods. =20 Contractor employees shall be required to conduct
counseling to = individuals,=20 couples, groups and families. The
Contractor shall have employees = available to=20 conduct group
counseling for abusive behavior treatment issues, = Marriage=20
Preparation and Marriage Enrichment, couples, and relocation issues. =
The=20 Contractor shall have employees available to conduct weekly
evening = support=20 groups or family skills counseling groups as
identified by the FSC = Director.=20 The Contractor shall be required
to conduct Domestic Violence = Intake/Assessment=20 screening
counseling for phone calls and walk-ins to the FSC as a = primary=20
task. The Contractor shall be required to conduct as requested and =
needed,=20 healthy lifestyle behavior skills educational counseling.=20
C.2 ANCILLARY TASKS. The Contractor shall maintain a log and computer
= input of=20 all individual, couples, family or group counseling
service cases. The = Contractor will maintain monthly Quality of Life
Management Information = System=20 (QOLMISNET) statistical data entry
input. The Contractor will maintain = and=20 follow FSC Accreditation
Counseling, and FSC Counseling Desk Guide = Quality=20 Control
Standards. The Contractor will maintain and submit a monthly =
projected=20 schedule to the Front Reception Desk and Clinical
Supervisor. The = Contractor=20 shall ensure all walk-in clients
complete FAP risk assessment and = intake=20 counseling forms. The
Contractor will also complete intake-counseling = forms=20 with
telephone counseling cases only if individual clinical counseling =
provided=20 is fifteen minutes or more. Each client shall be provided
FSC = Counseling=20 Evaluation forms at the conclusion of counseling
sessions or series. = All=20 initial counseling notes will be
maintained in the SOAP (Subjective, = Objective,=20 Assessment, Plan)
format. The Contractor shall make available all = records for=20
Quality Control (QC) Auditing to the FAP Supervisor or his/her =
representative. =20 All new records are required for QC auditing every
quarter. The = Contractor=20 shall maintain a current reference file
on CHAMPUS Tricare certified = providers=20 (counselors), or shall
refer to Health Care Finders for on base and off = base=20 referral.
For optimal referral of FSC clients, the information will = include=20
the counseling specialties for each person in the file. The Contractor
= shall=20 submit a year-end report consisting of client evaluations,
QOLMISNET=20 statistics, and work performance self appraisal with
recommendations = for=20 improvement of program and services.=20 C.3.
PERSONNEL QUALIFICATIONS. The contractor is required to provide =
one=20 full-time individual for each of the following categories and
shall = meet, as a=20 minimum, all of the educational and experience
requirements listed = below. =20 Resumes are required and are to be
submitted with your proposal. The=20 Contractor shall provide personnel
as identified by the Director, = Family=20 Service Center, NAU Scotia
as having the following qualifications. = Contractor=20 employees shall
possess a current state license to practice as Clinical = Psychologist
or accreditation to CISW or ASCW or AMFT and shall be a = Certified=20
Social Worker in the State of New York. Each contractor employee =
shall posses=20 a minimum of one year of clinical practice experience
(five years for=20 Counselor/Clinical Supervisor) working directly with
individual, = couples,=20 marital and group counseling. The Contractor
employees shall have = completed=20 clinical training through
seminars, courses, or workshops with = certificate of=20 completion in:
Brief therapy, family/marital therapy, problem or = solution=20 focused
therapy. These can be documented by transcript, training =
certificate=20 or testimonial letter by supervisor or trainer and must
be provided = prior to=20 performance under the contract. Contractor
employees shall have at = least one=20 year of counseling in providing
counseling services to individuals who=20 experience domestic violence
treatment. A letter by a supervisor or = consultant=20 providing
testimony of this clinical experience is required to provide = this=20
documentation. Such documentation must be provided prior to =
performance under=20 the contract. The Contractor must have adequate
professional liability = insurance and must agree to indemnify the
United States Government for = any=20 liability that may be assessed
against the United States Government = that is=20 attributable to any
action or omission of the provider. The Contractor = must=20 complete
State and Navy Criminal Investigation Background Checks in =
compliance=20 with DOD Child Care Directive and Public Law 101-647
Section 231. = (Required w ithin six months of contract award). C.3.1
COUNSELOR/CLINICAL SUPERVISOR C.3.1.1 Education. Master=92s Degree in
social work, marriage and = family=20 therapy, psychology, counseling
or other behavior science related field = from=20 accredited
institution. =20 C.3.1.2 Experience. Minimum five years of supervised
experience in = clinical=20 counseling. C.3.2 FAMILY COUNSELOR C.3.2.1
Education. Master's degree in social work in a school = accredited
by=20 the Council on Social Work Education.=20 C.3.3 FAMILY ADVOCACY
REPRESENTATIVE C.3.3.1 Education. Master's degree in social work in a
school = accredited by=20 the Council on Social Work Education.=20 C.4
GOVERNMENT FURNISHED MATERIALS. The Government will provide the=20
facilities, equipment, and materials in support of NFSC. NFSC is =
located at=20 277 Washington Street in Saratoga Springs, NY. Entrance
accessibility = to the=20 offices begins at 7:30 a.m. and continues to
4:00 p.m., Monday through = Friday,=20 except federal holidays. C.5
OWNERSHIP OF MATERIALS DURING THE COURSE OF THE CONTRACT. All =
materials,=20 included but not limited to standard operating
procedures, pamphlets, = flyers,=20 leaflets, program
manuals/materials, videotaped, formats for = presentations,=20 training
materials, books, journals, client records, case records, =
ancillary=20 material forms, curriculum developed/used/or purchased
during the = course of=20 this contract, shall be the property of the
Government at the end of = the=20 contract. Any exceptions to this
policy shall be granted only by the = FSC=20 Director and shall never
involve case records or associated materials. = The FSC=20 Director
shall have final authority over determination of FSC property=20
inventory items. C.6 PERFORMANCE REQUIREMENTS. The Contractor shall
provide the = required=20 services Monday through Friday, 7:30 =96 1600
excluding a 30-minute = lunch=20 break. The Contractor shall not be
required to provide services on = Federal=20 holidays. The ten (10)
paid Federal holidays for full time personnel = are as=20 follows: New
Year's Day; Martin Luther King, Jr. Day; President's Day; =
Memorial=20 Day; Independence Day; Labor Day; Columbus Day); Veteran's
Day; = Thanksgiving=20 Day; and Christmas Day. Services shall be
provided on State holidays = that do=20 not fall on a Federal holiday.
Services provided shall be conducted in = the=20 Family Service
Center, with the exception of work to military=20 communities/families
as requested and approved by the FSC Director. = When a=20 workday
includes an evening class or group, the 8-hour day may be = divided
into=20 morning and evening or afternoon and evening. If the evening
work = period is=20 scheduled, the total work period must be on base or
at the FSC. = Additionally,=20 the Contractor will be subject to
on-callstatus after normal working = hours for=20 identification of
child/spouse abuse cases, or child neglect cases.=20 Accordingly,
duties may necessitate occasional counseling after normal = working=20
hours. Overtime is not authorized. In these instances, contractor =
employees=20 may earn =93comp-time=94 for all hours performed outside
off normal = work hours.=20 Compensatory time must be taken at a time
agreed by the FSC Director, = but no=20 later than ten (10) days after
it is earned. No compensatory time = shall be=20 earned in the final
two (2) weeks of the contract year. C.7. USE OF FULL-TIME EMPLOYEES. In
order to ensure continuity of = care,=20 services shall be provided by
full-time employees to the maximum extent = practical. Substitute
personnel are authorized for scheduled absences = such as=20 employee
vacation. Substitute personnel shall comply with all contract =
qualification requirements. Resumes for substitute personnel shall
be=20 submitted to the FSC Director prior to performance by substitute
= personnel=20 under the contract. Contractor employees shall not
perform under the = contract=20 unless the FSC director has reviewed
the employees resume for = compliance with=20 contract requirements. C.
8. INVOICING REQUIREMENTS. The contractor may invoice twice a = month
at =BD=20 of the established monthly rate. The invoice at the end of
each month = shall be=20 accompanied by the following enclosures, which
are required for quality = control=20 and quality assurance: 1) Monthly
schedule completed =96 the initials = of client=20 appointments,
consultations, committee meetings, training sessions with = hours=20
worked, (2) Monthly schedule projection, (3) client/customer =
satisfaction=20 surveys completed, (4) Quality of Life Management
Information Systems=20 (QOLMISNET) client services documentation
verification, (5) Quality = Assurance=20 Clinical/Case Records Audit
Reviews conducted minimally in December, = March,=20 June and
September. Case records are to be submitted to Division =
Supervisor=20prior to those invoice submissions. The following FAR

Offeror's attention is specifically directed to = paragraph=20 (b)(11)
