Lutz VA Medical Center intends to issue a solicitation for a Community Based Outpatient Clinic (CBOC). The Aleda E. Lutz VA Medical Center will be soliciting proposals from qualified offerors, to provide Primary and Preventative Healthcare services, to include an existing established facility, equipment, supplies, lab, radiology, short-term pharmaceutical prescriptions and all administrative functions to fulfill the level of care necessary to meet the primary medical needs of veterans in and around the surrounding counties of Traverse City, Michigan. The assignment of veterans to the clinic will be determined by the Aleda E. Lutz VA Medical Center. Compensation shall be made through monthly capitation payments in accordance with the terms and conditions of the proposed contract. The contract expected to be awarded from this solicitation will include a Base Year and two (2) one (1) year renewal options which may by exercised at the option of the Government.
