SOL RFP 600-091-99 DUE 041599 POC Sharon Bailey, (562) 494-5896 The VA Network Business Center seeks Request for Quotation for a Firm Fixed Price contract (Commercial Item FAR Part 12) to provide orthotic appliances and related services for the Veterans Affairs Medical Center Long Beach, CA and the West Los Angeles Healthcare System, Los Angeles, CA. This will include custom made appliances and all necessary repairs to orthotic related appliances. Contractor facility must be readily accesible to the Veterans in Los Angeles County, Riverside County, Orange County, and San Bernardino County. Quotations received outside of these areas will not be considered in the bidding process. Contractor is required to have an American Board Certification (ABC) in orthotic and prosthetics. This will be a Three (3) year contract with one (1) base year and two (2) one (1) year options starting on or about 6-1-99.
