testing to be completed NLT 15 Aug 99. Quotes should be made on a per
test basis. All numbers are approximate and are not binding. The
Georgia Army National Gaurd guarantees at least 80% participation of
employees in the program. Authorization for tests can only be given by
the State Occupational Health Nurse. An authorization form must
accompany each employee at time of testing. Only tests authorized by
the Occupational Health Nurse will be billed to the Beorgia Army
National Guard. Test schedule will be coordinated through the
Occupational Health Nurse. Testing will only be performed Tuesday
through Friday. Testing will not begin before 0715 and will stop NLT
1700 each day. Contractor is to provide all hard copies of test results
along with a complete listing of results in a ledger form NLT 45 days
after all testing has been completed. Proposed schedule for testing is
to be provided to the Occupational Health Nurst NLT 15 May 99. Georgia
Army National Guard will not be responsible for any special equipment
needed by the contractor or for installation of any equipment.
Physician to review all test results for accuracy. Physician shall
report any possibly serious abnormal result as soon as reasonable but
not more than 3 weeks from time of test. Physician to perform on site,
hands-on examination of employees on the respirator program only. At
the discretion of the contractor, on site physician may set up a
separate visit to the site. Examinations must however be completed NLT
31 Aug 99. One consolidated invoice for all sites visited should be
submitted to the Occupational Health Nurse for review and payment.
VISION SCREENING: Multiphasic vision screening equipment (Titmus type),
able to measure the following primary visual skills. Visual acuity for
distance and near; Laterial phorias for distance and near; Depth
Perception; Color Perception; Vertical phorias for distance and near;
and Peripheral Vision. Technician performing the vision screening shall
be qualified technicians and shall produce proof of qualifications.
PULMONARY FUNCTION TEST (PFT) WITH CLEARANCE: Including as a minimum of
determination of the forced expiratory volume in one second (FEVI) and
the forced vital capacity (FVC). This shall include clearance by a
physician to wear a respirator. Physician will make on sit visit to
locations having employees that are on the respirator program and
perform a hands-on exam of those employees on the respirator program.
Exact numbers and locations will be provided upon request. Technician
performing the PFT shall be certified and produce proof of
certification. AUDIOGRAM: Audiogram with range of 500 to 6000 and zero
reference. No age adjustment in calculating results. Technician
perform the Augiogram shall be certified and produce proof of
certification. BLOOD CHEMISTRY: Lead and ZPP (lead and zinc
protoporhyrin levels), SMA/CBC/UA (kidney and liver function package),
and PSA (only employees over the age of 40). Locations and approximate
