Attachment #5, entitled "Technical Proposal Evaluation Criteria" is amended by adding the following "Technical Factor" to "Special Technical Factors" as follows: The offeror's past performance with emphasis placed on expertise of key personnel. The offeror shall identify no more than three (3) contracts over the last three (3) to five (5) years documenting past performance related to the current contract. The offeror shall provide the contract number, agency, point of contact and phone number and the contractor's assessment of their past performance relating to these contracts.
