combined synopsis/solicitation for commercial services prepared in
accordance with the format in FAR subpart 12.6 as supplemented with the
additional information included in this notice. This announcement
constitutes a solicitation; proposals are being requested and a written
solicitation will not be issued. This solicitation, No. F02604-99-Q0161
is issued as a Request for Quote (RFQ). This solicitation document and
incorporated provisions and clauses are those in effect through
Federal Acquisition Circular 97-2. Notice: This acquisition is
restricted totally as a small business set-aside; SIC: 8999; Size Std:
$5 million dollars. All responsible sources may submit a quote, which
shall be considered. MANDATORY SITE VISIT: A MANDATORY SITE VISIT IS
SCHEDULED FOR 19 Mar 1999 at Gila Bend AFAF Main Gate. Please arrive no
later than 10:00 AM for access. Item: DESCRIPTION OF WORK The Asbestos
abatement project is located in southwestern Arizona approximately 3
miles south of Gila Bend, off of StateRoute 85 on Gila Bend AFAF. The
project is located in Maricopa County. DESCRIPTION OF WORK -- Abate
asbestos containing material (ACM) and lead based paint in facilities
42, 61, 62, 90, 93, 95, 100, 101, 102, 103, 104, 105, 106, 107, 108,
109, 110, 111, 112, 113, 114, 115, 116, 117, 118, 119, 120, 121, 122,
124, 126, and 128. Dispose of ACM at an approved landfill. Comply with
all County, State and Federal standards for asbestos removal. EXISTING
SITE CONDITIONS: The project is situated within federal lands on an
active Air Force installation. There are overhead utilities and
adjacent facilities at the site. Independent companies and the U.S. Air
Force still utilize Gila Bend AFAF for operations. Considerations shall
be made to preserve their ability to function al and operational.
GOVERNMENT FURNISHED REPORT: Asbestos Sampling Report. The Contracting
Officer is providing the contractor with an Asbestos Sampling Report
based on samples taken by the Government and analyzed at a testing
laboratory. This report constitutes a representation of what is
believed to be a complete identification of the asbestos containing
material associated with a facility or site, and be definitive to the
degree that the contractor may use the report to determine initial
project requirements. This report is not a certification of total scope
identification. The asbestos report can be obtained by reviewing the
requirement posting on the EPS web site http://www.eps.gov .The
contractor shall remain responsible for notification to the contracting
officer (CO) of any additional suspected asbestos-containing materials
discovered during the execution of the contract. Monitoring: The
government may employ a Certified Industrial Hygienist (independent of
that required to be employed by the contractor) to monitor air
quality, project procedures, and to ensure the contractor is in
compliance with applicable federal, state, and local regulations.
PEFORMANCE PERIOD: Thirty (30) days from Notice to Proceed. RECORD
KEEPING: The contractor shall provide to the CO the written records or
manifests verifying receipt of the quantity received (volumes and
weights, as necessary) of each shipment at each disposal facility, and
verification of proper disposal. When notification of receipt of any
shipment is not received within ten (10) days of departure of the waste
from the site, the contractor shall immediately notify the CO. Also,
the contractor shall contact the disposal facility to determine the
status of the shipment and resolve the discrepancy. The clauses at: FAR
52.212-4, Contract Terms and Conditions -- Commercial Items applies to
this acquisition, Reference FAR 52.212-4 para (c) is hereby tailored
as follows: (c) Changes. Changes in the terms and conditions of this
contract may be made only by written agreement of the parties with the
exception of administrative changes such as changes in paying office,
appropriations data, etc., which may be changed unilaterally by the
Government. Reference 52.212-4 paragraph (g)(8) is to read as follows:
(g) Invoices will be handled in accordance with the Prompt Payment Act
(31 U.S.C. 3903) and Office of Management and Budget (OMB) Circular
A-125, Prompt Payment. Contractors are encouraged to assign an
identification number to each invoice; The clauses at: 52.212-5
