The VA Northern Indiana Health Care System, Marion, Indiana has a requirement for a Community Based Outpatient Clinic (CBOC) to cover primary care services in the Muncie/Anderson, Indiana area(Delaware and Madison Counties). The contractor shall provide for the delivery and management of continuous primary and preventive medical care of eligible veterans. The contractor shall provide health care clinical providers, medical facilities, medical equipment, supplies, laboratory and imaging services, and all administrative functions to fulfill the level of care and range of services. Compensation will be made through monthly capitation payments in accordance with the terms and conditions of the proposed contract. The period of performance will be for a base year with two on-year option periods, which may be exercised at the option of the Government. The SIC is 8011, $5.0 mil.
