Subj: Sources sought for hospital supply items This is a Market Survey to locate potential sources for the standardization of various hospital supply items. The Department of Veterans Affairs (VA) desires to offer its customers, on a national basis, the best possible products and prices and to offer its contractors more opportunities within the system. The intent of the standardization program is to establish national preferred sources for hospital supply items to standardize VA's purchasing of products. Bids/Offers are not being solicited at this time. A formal solicitation or request for quotations will be issued at a later date. Companies interested in having their items evaluated for possible inclusion in the standardization process should carefully review the item descriptions listed below. If your company can provide any of these items, please forward samples of all items available from your company that meet the product descriptions and any corresponding product literature to: Department of Veterans Affairs Item Management Division (901S) ATTN: Mr. Carl Hunter Building 37 First Avenue -- One Block North of Cermak Road (22nd Street) Hines, IL 60141 Companies submitting samples are also encouraged to obtain FSS contracts from the respective government contract office. The Environmental Program Service Product Review Group, who have expertise and knowledge of the products, will review and technically evaluate any samples submitted. VA deems the members of this group to be experts; therefore, they are qualified to determine which products are acceptable. Samples and product literature must be received no later than April 16, 1999. Companies not submitting samples by the above date may be eliminated from consideration on any subsequent procurement action for these items. Product Descriptions and criteria for items being considered at this time are as follows: TOILET PAPER-QUANITY OF SAMPLES NEEDED- 6 ROLLS EACH -- DISPENSERS -- 1 EACH SIZE AND TYPE 1. Standard, Mini and Jumbo Rolls, white, 2. 1 and 2 ply. 3. Include approx. sheet size, feet/rolls, rolls per case, approx. ft/case. 4. Must be available in premium and economy grades, meet EPA standards. 5. Must be made from recyclable materials. 5. Must have dispensers and training provided for loading of dispensers. PAPER TOWELS-QUANITITY OF SAMPLES NEEDED-6 EACH SIZE/TYPE-1 DISPENSER EACH SIZE AND TYPE 1. C-fold, Single fold, Multi-fold, High Footage Roll, 2. Colors -- brown and white 3. 1 ply 4. Include approximate sizes, feet/rolls, rolls per case, approx. ft/case. 5. Must be available in premium and economy grades, meet EPA standards. 6. Must be made from recyclable materials. 7. Must have dispensers (new and replacement) and training provided for loading of dispensers. THERMAL BLANKETS- QUANTITY OF SAMPLES NEEDED -- 2 EACH 1. Fiber content and mix 50/50 or 65/35 (poly-cotton) 2. Provide Weave, thread count, weight and warmth 3. Solid color selection 4. Launderability (instructions) 5. Labeling (industry standard) 6. Display apparatus to determine refill requirements 4. Hygienically sealed dispensing units 5. Complete skincare program/body (hair and body) cleaner, moisturizing, skin protection, hand and skin cleaner, anti-microbial. HAND SOAP AND BODY SHAMPOO -- QUANTITY OF SAMPLES NEEDED -- IF YOU HAVE VARIOUS SIZES WITHIN A PRODUCT, 2 EACH SIZE, ONE SIZE PER PRODUCT -- 6 EACH 1. Lotion soap-removes light soil that helps reduce skin dryness -- leaves skin soft -- will lather quickly -- proper preservatives to prevent product contamination. 2. Body washing shampoo-used for routine gentle bathing, shampooing -- rinses clean and may be used for routine hand washing -- Must leave skin and hair soft and clean. 3. Antimicrobial soap -- Non-irritant anti-micro agent with documented evidence against gram positive and gram negative bacteria.
