project provides for whole house revitalization of Quarters 36, Bldg.
No. 83 located at 36 Makalapa Drive, Naval Base, Pearl Harbor, Oahu,
Hawaii. The work includes interior renovation of the existing kitchen,
living room, dining room, family room, bathrooms, bedrooms, storage
room, utility room, and installation of a central air conditioning
system. Mitigation of asbestos and lead paint hazards and repair of
termite damages to the interior of building. The electrical system will
be upgraded to provide energy efficient lighting, ground fault circuit
interruption devices and appropriate service panels, fixtures, and
outlets to meet current codes in dwelling. The plumbing system will be
upgraded and water saving devices will be provided. PROCUREMENT METHOD
IS THE SOURCE SELECTION PROCESS AND THE FOLLOWING EVALUATION FACTORS
WILL BE USED IN THE EVALUATION OF PROPOSALS: 1. FACTOR A, PROPOSER'S
PAST PERFORMANCE INFORMATION (PPI): Past performance based on customer
satisfaction as demonstrated on renovation work similar to this
project and similar compressed schedule within the past five (5) years.
List project title, location, contract agency/owner, date
award/complete, award amount/final amount, brief description, point of
contact (fax/telephone nos.). SUBELEMENTS: (1) Building user's
satisfaction for quality of workmanship, timely completion of work,
construction management satisfaction, and reasonableness of cost.
Provide contractor's performance evaluation, recognition of awards, and
letters of appreciation. (2) Contractor's safety record and Experience
Modification Rate (obtainable from company's insurance agent) for
current year and last five years. (3) Contractor's claim and disputes
rate; contract modification and cost growth rates. 2. FACTOR B, KEY
PERSONNEL INFORMATION (KPI): Information for Project Manager, Project
Superintendent, and Quality Control Manager who will be assigned to
this contract and who performed work relatively similar to this
project. 3. FACTOR C, PROPOSED MATERIAL/EQUIPMENTLIST (MEL). Proposed
material and/or equipment for this project. PRICE IS ALSO EVALUATED AND
CONSIDERED AN EQUAL VALUE IN DETERMINING BEST VALUE. This is a firm
fixed price contract estimated: $250,000 to $500,000. Solicitation is
unrestricted for SIC 1521 Annual Size $17.0 M. RFP available o/a 5
April 99. A letter requesting this RFP shall be mailed to: Defense
Automated Printing Service, PO Box 126, Pearl Harbor, Hawaii
96860-5120. There is a non-refundable charge of $87.90. Checks shall be
made payable to "Superintendent of Documents". If using Visa,
MasterCard, or Discover Card, be sure to include the cardholder's name,
account number, and expiration date in your letter. The letter must
also state your company name, address, telephone number, facsimile
number, RFP No. and project title, and whether the RFP will be picked
up at Regional Officer In Charge of Contracts, Construction Contracts
Division, 4262 Radford Drive, Building 62. Please indicate whether
prime, subcontractor, or supplier. On the front of envelope, type RFP
No. Allow at least ten days for receipt of solicitation. Companies
wanting the RFP sent via air express service must furnish their express
service's air bill label with the recipient's name, telephone no.,
company name, address, company's account no., and type of delivery
filled in. Failure to provide any of the above information and check,
as indicated, may delay the processing of the RFP and is at no fault of
the Government. INDIVIDUAL CHECKS AND REQUESTS ARE TO BE SUBMITTED FOR
EACH SOLICITATION. Plan holders list may be obtained from the DAPS web
page at http://www.hawaii.wnbt.daps.mil. If unable to access through
the web page, you may fax a request for plan holder's list to (808)
474-7316. Any request for solicitation(s) received within 10 working
days prior to the proposal closing date will be processed in accordance
with the above procedures. However, there is no guarantee that the
recipient will receive the solicitation prior to the closing date.
