items prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; quotations are being
requested and a written solicitation will no be issued. Solicitation
RFQ V15-99-87 is issued a request for quote and incorporates provisions
and clauses are those in effect through the Federal Acquisition
circular (FAC) 97-06. The Standard Industrial Classification Code (SIC)
3826 Small business Size Standard is 500 Employees. The Department of
Veterans Affairs, VA Heartland Network 15 Business Office, Leavenworth,
Kansas, intends to procure the following item: 1.) 1 unit of a model
4890*TN Sakura VIP E150 Benchtop Tissue Processor, capacity equal to
150 cassettes, Solution volume requirements 2.2 liters, memory 9
programs with individual self-calculating delay modes, interfaces
external alarm connection, retort temperature ranges stations 1 to 10
equal to 35 degrees C to 50 degrees C, Stations 11 to 14 equal to 45
degrees C to 70 degrees C, oven temperature remaines 2 degrees C above
set temperatures of stations 11 to 14, Dimensions 100.0 cm (39 in) x
58.0 cm (23 in) x 66.9 cm (26 in), electrical is 115 VAC @ 10.5 A.60
Hz. All-station, operator-selectable control, uniform reagent
temperatures throughout the retort during processing, gentle tidal
agitation, paraffin cleansing, full-screen display, comprehensive,
easy-access control panel, and basket design for upright placement of
cassette specimens. Delivery of unit shall be on or before 30 days ARO.
Delivery address is VAMC, Warehouse, Building 41, 4101 S. 4th St.
Trafficway, Leavenworth, KS 66048. All responsible sources may submit
a quote, which will be considered by this agency. Responses shall be in
writing and can be FAXED to (913) 758-6495 to the attention of the
contracting officer. The following FAR clauses apply to this
solicitation: FAR 52.212-1, Instructions to Offerors, Commercial Items
with the addendum FAR 52.216-1 type of Contract (APR 1984) "The
Govenment contemplates award of a firm fixed price contract resulting
from this solicitation," FAR 52.212-2, Evaluation-Commerical Items,
"Award will be made to the responsive offeror which provides the lowest
offered price and meets all the terms and conditions set forth in this
solicitation." Offerors will be required to provide past performance
documentation as part of the evaluation for award of best value to the
government. All offerors will be required to provide a completed copy
of the provision FAR 52.212-3 Offeror Representations and
Notifications-Commercial Items. FAR 52.212-4 Contract Terms and
Conditions-Commercial Items. FAR 52.212-5 Contract Terms and Conditions
Required to Implement Statutes or Executive Order-Commerical Items. The
Standard Commercial Warranty is required. Offers must be submitted 6
calendar days from the publication of this notice and should be
