Solicitation N00033-99-R-8012 is issued as a Request for Proposals. (iii). The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 97-08. (iv) . The solicitation is unrestricted and is not set-aside for small business concerns. OPTION PERIODS ONE THROUGH FOUR: CLINS 0002 THROUGH 0005 Repeat CLINS/SUBCLINS of CLIN 0001 for each option period. ALL PERIODS -- CLIN 0006: TRAVEL REIMBURSEMENT JOINT TRAVEL REGULATIONS (The Contractor shall be reimbursed for travel at his actual costs when applicable, not to exceed the limits set forth in the JTR. Reimbursement will not be made for travel performed for the convenience of the contractor. Per Diem shall be payable only when the Contractor's personnel are required to be away from the Contractor's facilities or the employee's home overnight). (vi). Description: This solicitation is for services in support of Military Sealift Command vessels with INTERSPIRO Model 9030 SPIROLITE High Pressure Self Contained Breathing Apparatus (SCBA). Service shall be required for a maximum of forty-two (42)SCBAs, and forty-two (42) spare cylinders per vessel. It is anticipated that the maximum of Fifty (50)Vessels will be requiring servicing each year. A complete SCBA unit is a fully assembled breathing apparatus that can be immediately donned and used in a contaminated air environment and/or in fire fighting by the operator. The components of a fully assembled SCBA unit shall include the facemask, back mounted air cylinder, harness and straps, regulator, air hoses, gauges and valves. (vii). The Contractor shall provide the services/parts listed below. All services shall be performed in accordance with inspection and testing set forth by the manufacturer and certification requirements of the National Institute for Occupational Safety and Health (NIOSH), the Mine Safety and Health Administration (MSHA), the National Fire Protection Association (NFPA) and other applicable regulatory agencies. A Notional Ship Schedule will be provided upon request. a. Annual Service ("AS") (every twelve months, plus or minus two months): inspection, testing, maintenance, repair and recertification of SCBA units. Service shall be conducted in port and shall be completed within a three day period (CLINS 0001AA; 0002AA; 0003AA; 0004AA; 0005AA). b. Annual Service with Hydrostatic Test ("ASH") (each vessel every three years, plus or minus two months): annual service plus a cylinder hydrostatic testing of cylinders. Testing shall coincide with annual service visits. Service for annual inspection and triennial hydrostatic testing shall be conducted in port and shall be completed within a four day period (CLINS 0001AB; 0002AB; 0003AB; 0004AB; 0005AB). c. Travel for ports named in the schedule shall be inclusive of all transportation, per diem, and miscellaneous expenses incurred by the contractor. Travel shall be payable at the Annual Service rate, or at the Annual Service with Hydrostatic Test rate specified in CLINS 0001BA-BY through CLINS 0005BA-BY. Travel for Ports not otherwise specified in the schedule shall be reimbursed in accordance with the Joint Travel Regulations. d. Provision of all items used to inspect, test, and service onboard SCBAs including repair and replacement parts are required to meet manufacturer's recommended schedule for equipment parts. This shall include shipping, receiving and all transportation costs associated with parts, components, spare cylinders, special tools, testing materials or other equipment required to perform the services (Costs included in CLINS entitled "Annual Service" and "Annual Service with Hydrostatic Test"). e. Contract Data Requirements as follows: (i) Annual Fit for Service Certification: Contractor shall provide a written certification of acceptability of all-shipboard SCBA units and components, as intended to each ship's master prior to departure. The Contractor shall also provide written certification of acceptability to COMSC N72 at "COMMANDER MILITARY SEALIFT COMMAND, WASHINGTON NAVY YARD, 914 CHARLES MORRIS COURT SE, CODE N72PC2, WASHINGTON DC 20398-5054" within ten working days after completion of each ship's annual SCBA service visit. (ii) Identification of Service Personnel: The Contractor shall provide advance notice of personnel changes to the Contracting Officer. Notice shall include proof of qualifications to perform the intended service. f. Ordering: Ordering Officer(s) will be designated at Contract Award. Orders may be issued orally, by facsimile, or by electronic commerce, however they shall be followed up in writing. (viii). The Contractor shall meet the following minimum requirements: a. Certification by the Department of Transportation for hydrostatic testing services. b. Certification by the Manufacturer as an authorized Service Representative. Capability to conduct a minimum of two simultaneous ship visits worldwide for annual servicing. (ix). DATE AND PLACE OF DELIVERY: Base contract term is 30 APR 1999 TO 29 APR 2000 or "One year from the Contract Award" with the Government's option of four (4) additional twelve (12) month periods. (x). FAR 52.212-1, "Instructions to Offerors-Commercial" (Provides instructions for responding to this solicitation). Offerors shall also provide the following information: 1. Price(s) for each CLIN (Offerors shall obtain the schedule of CLINs with estimated quantities from the POC in paragraph XIX below); 2. Description of qualifications, knowledge and experience with respect to testing and inspection services and cylinder hydrostatic pressure testing of INTERSPIRO MODEL 9030 SPIROLITE SCBA; 3. A Summary stating the offeror's general understanding and approach to complying with the requirements of this solicitation. 4. Copies of offerors' procedures, checklists, and associated instructions for performing required servicing and testing. Procedures offered which are in excess of manufacturer's recommendations shall be incorporated in the Contract. 5. Plan to accomplish simultaneous servicing in different geographic locations worldwide. Plan must, at a minimum, address personnel qualifications, equipment and location/capabilities of facilities. 6. Description of ability to provide required repair and replacement parts, through inventory or other resources. (xi). FAR 52.212-2, "Evaluation Commercial Items: (Provides evaluation criteria for Award), INSERT THE FOLLOWING FILL-INS IN PARAGRAPH (A) OF THE PROVISION: "The following evaluation factors shall be used to evaluate offers: 1. Technical capability of the offeror to provide the required services for the Interspiro Model #9030 Spirolite SCBA. 2. Price; and 3. Past Performance. Technical and past performance, when combined, are approximately equal to price. An evaluated price will be developed by adding the total (firm period plus options) price of Service (Annual Service and Annual Service with Hydro Test) and Travel for Service Calls (Annual Service and Annual Service with Hydro Test). (xii). OFFERORS MUST COMPLETE AND RETURN WITH THEIR PROPOSAL the information required by FAR 52.212-3 ("Offeror Representations and Certifications Commercial Items"). (xiii). FAR 52.212-4 "Contract Terms and Conditions Commercial Items" applies to this acquisition. (xiv). FAR 52.212-5 "Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items" applies to this acquisition, including the following applicable clauses from paragraphs (b) and (c): 52.203-6, 52.219-8, 52.219-9, 52.219-23, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-9, 52.239-1, 52.222-42, and 52.222-43. (xv).
