This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The solicitation number is: 549-62-99 and is issued as a Request for Proposal (RFP). SCOPE Veteran's Information Verification Project. Successful offeror will be given 25,00 0 names and addresses from a department of Veterans Affairs database. Successful offeror will provide VANTHCS a minimum of 5,000 verified names, addressees and good phone numbers for veterans from that original list of 25,000. Verification methodology is up to the successful offeror, but must include at least one telephone contact with the veterans or their spouse to be considered a verified name for inclusion in the list of 5000 successful offeror will have 45 days from the date they receive the list of 25,000 unverified names to complete the verified list of 5000 names. Names will be furnished on a disk in ASCII format. ASCII file provided by VA will contain the name, last known address; service-connected disability percentage and service connected disability code. The report back to VANTHCS of 5000+ verified veterans will be in excel and sortable by the following fields: Last name, city, County, Zip Code, Percent of Disability and service connected disability code. EVALUATION FACTORS Offers will be evaluated by the following evaluation factors with technical proposal worth twice as much as past performance and past performance worth twice as much as price. Each offeror must submit the following information. (1) TECHNICAL PROPORSAL (a) Method(s) of verification used for obtaining information. Cost (labor, overhead and fee) Shall be included in this section. This information shall support the price proposed. (b) Ability to meet required delivery time shall be defined by brief narrative. (2) PAST PERFORMACE (a) List of clients minimum of three to include name, address, and phone number of Contact person. This list should only include clients serviced over the past two years.
