Contractor to provide one General Dentist to perform dental services, 5 days a week, as detailed in the Performance Work Statement at the Dental Activity, Fort Carson, Colorado. The Government will furnish all facilities, supplies, equipment, and ancillary staff necesary to deliver routine dental services. Contract Period: 17 May 1999 through 30 September 1999, with four one year options. The solicitation package is for commercial items prepared in accordance with the format in Federal Acquistion Regulation (FAR) Part 12, as supplemented. This solicitation will be issued as a Request for Proposals (RFP) o/a 9 April 1999. Proposals received from contractors will be evaluated for past performance, previous experience, and technical capabilities. The proposed contract will be 100% set-aside for small business (SB). For this acquistion, the SB size standard is $5 million in total revenues. Interested small business concerns may submit a written/facsmile requests. Written requests must have the following: business name, business address, point of contact, tax identification number, corporate status, CAGE code number, DUNS number, and a signed statement as status as a small business.
