MacDill AFB located in Tampa Florida has a requirement for a Clinical Dietician. Services to be performed on site. Contractor will provide care for the 6th Medical Group mainly to beneficiaries participating in the MacDill 65 Demonstration Program. Work to include, nutritional screening, assessment, calculating nutritional requirements, constructing patient plans with solutions and documents in education to both inpatients and outpatients at the Medical Treatment Facility (MTF). Provide individual and group nutrition counseling for patients with diabetes, hyperlipidemia, hypertension and obesity. Designs new pamphlets/handbooks, outreach briefings and classes for various nutrition programs. Conducts special reviews to gather clinical information on patients for reporting. The range of number of patients is approximately 1700-2300 per year per year, 1 Full Time Equivalent (FTE). Age group of patients is 65 and older. Program is estimated to last 3 years from september 1998. A bachelor's or Master's Degree in Nutrition/Dietetics, Certificate of Completion of Dietetic Registration of the American Dietetic Association, valid license to practice, completion of a Gneralist Internship Program, Certificate of Current Physical Exam and current BLS Certification are required to be considered. Hours of operation are Monday through Friday, 0730-1630 (7:30 a.m. -- 4:30 p.m.) excluding Federal holidays. The MTF is responsible for scheduling patient appointments. This solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 97-5 and will be awarded under Federal Acquisition Regulation (FAR) Part 12 Acquisition of Commercial Items. Anticipated Award Date is 26 April 99 and anticipated performance start date is 1 May 99. See Note 1. The resultant purchase order will be firm-fixed price. (a) The Government will awarda purchase order with one (2) two option years resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award will be based on Performance Price Trade off (PPTO). Technical and past performance are of greater value than price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. (c) A written notice of award or acceptance of an offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Evaluation of options shall not obligate the Government to exercise the options.
