The services will not cover the care and treatment of the infant. These services will cover the following counties in Utah, Nevada, Wyoming and Idaho: Utah-Box Elder, Cache, Rich, Weber, Morgan, Davis, Salt Lake, Summit, Wasatch, Tooele, Utah, Duchesne, Daggett, Uintah, Juab, Carbon, Emergy, Sanpete, Millard, Sevier, Beaver, Piute, Wayne, Iron, Washington, Garfield, and Kane; Idaho-Clark, Fremont, Jefferson, Maddison, Teton, Bonnville, Bingham, Power, Bannock, Caribou, Bear Lake, Franklin, and Onelda; Wyoming-Teton, Sublette, Lincoln, Sweetwater, and Uinta; Nevada-Elko and White Pine. Split awards may be made for different counties to ensure coverage of all areas. The contract will be for a base year with two one-year option periods.
