65 -- IMAGE INTENSIFIER AND MONITOR UPGRADE SOL RFP 600-118-99 DUE 041699 POC Brian C. Van Orman (NBC/MP) (562) 961-8043 This is a combined synopsis/solicitation. The VA Long Beach Healthcare System, Long Beach, California intends to negotiate a sole source contract with Siemens. The Request for Proposal #RFP 600-118-99-BV entitled "Image Intensifier and Monitor Upgrade" is hereby issued for commercial items and is prepared in accordance with the format Subpart 12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC 97-11. The SIC Code is 3844 and the small business size standard is considered 500 employees. This CBD announcement constitutes the only solicitation issued. A separate written solicitation document will not be issued. Siemens shall provide four (4) monitors HM 44cm (Simomed) and one (1) Siemens imaging tube 11 Sirecon 40-4 HDR. The four monitors comprise an integral part of the Siemens Videomed S TV system currently installed at the facility. Due to compatibility requirements, Siemens is the only brand that will not compromise the image quality of the complete medical video chain. Other systems cannot provide the same visual brightness and so, may compromise the clinical evaluation of the images. Siemens makes the Sirecon 40-4 HDR image intensifier and the Siemens image intensifier tube insert is the only one that would fit the current system. Offeror shall certify that the components listed above are Y2K compliant. Delivery terms shall be FOB Destination (unless vendor states otherwise) to the VA Long Beach Healthcare System, Warehouse Building 149, 5901 East Seventh Street, Long Beach, CA 90822. This will be a firm fixed price contract. Offeror shall submit a complete copy of provision FAR 52.212-3, Offeror Representation and Certification -- Commercial Items (Jan 1997).
