Q -- GENERAL DENTIST SOL RFP 546-20-99 DUE 043099 POC Lorna S. Barker, Contracting Officer 305 324-3285 Contractor will provide for a 0.5 full time equivalent employee (FTE) General Practice Dentist. The contract individual will be expected to treat patients referred to the VAMC Miami, FL by the Department of Defense (DOD). In the absence of the DOD patients to treat, contractor will provide treatment to eligible VA beneficiaries. The contract dentist will provide diagnostic, preventive, restorative, endodontic, periodontal and prosthodontic (fixed and removable) treatment procedures. The Government will furnish all facilities, supplies, equipment, and ancillary staff necessary to deliver dental services. The contractor must have all licenses, accreditation's and certificates required by law. The contract period is from May 4, 1999 through May 3, 2000 with a one-year renewal option. The solicitation package is prepared in the commercial item format (FAR 12). The proposed contract is 100% set aside for small business. For this acquisition, the small business size standard is $5 million in total revenues.
