Provide a maximum of 48 hours turn-around time for all reports. Provide each exam site with required collections, labeling, requisitions, packaging, and reporting materials required by the specified laboratory specimen testing procedures. Military medical personnel will perform the collection of all specimens. Specimens will be collected during weekend training assemblies and 2-week Annual Training periods as published by the Tennessee Army National Guard. Each site will have a coordinator to place calls against the contract and coordinate with the contractor for pick-up and delivery dates and times. Testing procedures are as follows; Prior Service Enlistment Exams-Urine Drug Abuse Screen 10 (Qualitative Analysis), Blood Alcohol (ETOH). Periodic Exams require total cholesterol (serum), HDL (serum), Glucose (fasting blood sugar)(serum), Hemoglobin (whole blood-EDTA). Estimated quantities for Prior-Service Exams at Smyrna, TN 200ea, Lavinia, TN 150ea, Chattanooga, TN 100ea. For Periodic Exams the estimated quantities are as follows; Smyrna, TN 700ea, Lavinis, TN 600ea, Chattanooga, TN 400ea, Cookeville, TN 100ea, Alcoa, TN 100ea, Knoxville, TN 300ea, Johnson City, TN 400ea, Nashville, TN 10ea. The successful contractor must be a national accredited, Clinical Laboratory Information Act (CLIA) approved for complex testing procedures and a TRICARE health services provider.
