synopsis/solicitation for commercial items prepared in accordance with
the format in FAR, Subpart 12.6 as supplemented with additional
information included in this notice. This notice constitutes the only
solicitation; quotes are being requested and a written solicitation
will not be issued. This solicitation is being issued as a Request for
Quotation number RFQ 688-8984-0035. The solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular number 97-04. Bid Item 1: Topcon Ophthalmic

Graphics Adapter), SONY PAL 950P Color 3-Chip CCD Camera, Color
ImageCapture/Image Display Card, 17" COLOR UVGA Monitor, Modem (56K
BAUD), Windows NT+ IMAGEnet 2000 (32 Bit) WIN Software, Communication
and Utility Software, CD Archiving 4x Write-6x Read, CD Reader 24x,
Relay Lens for Retinal Camera TRC-50VT, Battery Backup (UPS) and Line
Controller, System Table and necessary wiring. VAAR Clause 852.210-77
Brand name or equal applies to this solicitation. Bid Price to include
installation and training, FOB destination. The SIC code for this
solicitation is 6540. The small business size standard is 500
employees. Acceptance time will be 60 days from bid opening date. The

to acceptance of the goods or services provided under this contract it
will be necessary for the government to conduct a technical acceptance
inspection. This inspection will be completed within 5 calendar days of
delivery of the goods or services provided under this contract. A
written notice of award or acceptance of offer, mailed or otherwise
furnished to the successful bidder within the time for acceptance
specified in the offer shall result in a binding contract without
further actions by either party. Bidders responding to this
announcement shall submit a price for this system in accordance with
FAR 52.212-1 to include: a completed copy of provision 52.212-3
(Offeror Representations and Certifications), a completed copy of VAAR
852.210-71 (Guarentee) and 852.219-70 (Veteran Owned Small Business)
and, if bidding other than brand name, the manufacturer, model or
catalog number, and specifications of the equal. Bids to be submitted
