813-972-7593 This is a combined synopsis/solicitation for commercial
items prepared in accordance with the subparts in Federal Acquisition
Regulation 12.6, as supplemented with additional information included
in this notice. This announcement constitutes the only solicitation;
bids are being requested and a written solicitation will not be issued.
Solicitation number Request for Proposal (RFQ) 673-72-99 is assigned
and shall be referenced on any proposals. This solicitation document
and incorporated provisions and clauses are those in effect through
Federal Acquisition Circular 97-011. The Standard Industrial
Classification Code is 8071and the small business size standard is 5
million dollars. The contractor will provide as needed MRI services at
the contractor's place of business, which must be located within 30
miles of the Brevard Outpatient Clinic, 2900 Veterans Way, Melbourne,
Florida 32940. Period for services will be from date of award through
September 30, 1999. This contract will have four option years tobe
exercised individually. The MRI services will be provided for patients
referred by the Department of Veterans Affairs Brevard Outpatient
Clinic. Contractor shall provide services Monday -- Friday (excluding
federal holidays -- Martin Luther King Day, Presidents Day, Memorial
Day, Independence Day, Labor Day, Columbus Day, Veteran's Day,
Thanksgiving Day, Christmas Day, New Year's Day) from 8:00 A.M. -- 4:00
P.M. The following services will be provided: A routine request shall
be performed within 96 hours. An emergency request shall be performed
within 8 hours. A final written report shall be provided within 48
hours. The contractor shall provide a computer disk of the MRI results.
The contracting officer shall provide the name and telephone number of
the contact person for this contract. The contractor shall be
responsible for protecting the personnel furnishing services. To carry
out this responsibility, the contractor shall provide the following
for these personnel: worker's compensation; professional liability
insurance; health examinations; income tax withholding; social security
payments. The parties agree that such personnel shall not be considered
VA employees for any purposes and shall be considered employees of the
contractor. Invoices for services will be submitted by the contractor
monthly in arrears to the following address: Department of Veteran's
Affairs Outpatient Clinic, Attn: Al Sala, 2900 Veterans Way, Melbourne,
FL 32940. The personnel assigned by the contractor to perform the
services covered by this contract shall be licensed in a state,
territory, or commonwealth of the United States or the District of
Columbia. The qualifications of such personnel shall be subject to
review by the Chief, Medical Officer. Bids will be considered only from
providers, who are regularly established in the business called for and
who are financially responsible and have the necessary personnel and
facilities to furnish service in the volume required for all the items
under this contract. Successful bidder shall meet all requirements of
Federal, State, County, or City codes regarding operations of this
type of service. Clinical or other medical records of VA beneficiaries
treated by the contractor will be forwarded to the VA. Initial
duration for this contract shall be through September 30th, 1999;
however, there will be four option years from October 1st, through
September 30th. The solicitation number (RFQ number) shall be listed on
the outside of your response for identification. Proposals shall be
submitted on an "all or none basis." Partial proposals shall be
rejected. All potential offerors shall be available to perform services
immediately. Offerors shall provide a complete set of provisions with
proposal found at FAR 52.212-3, Offeror Representation and
Certifications-Commercial Items. Provision and clauses found at
52.212-1, 52.212-4, 52.212-5 and 852.210-77, also apply. In addition
52.217-8 "Option to extend services", 52.217-9 "Option to extend the
term of the contract", VAAR 852.270-4 "Commercial advertising", and
852.271-70 "Nondiscrimination in services provided beneficiaries" apply
to this contract. Proposals shall be submitted to the address listed

Dubuis (90C). The government shall not be held responsible for quality
