Cooper, Contracting Officer, 317-554-0047 This is not an RFP or IFB. -- Sources sought to provide veterans with accessible, network based home health services. The services to be provided are in the Indianapolis, IN, VA Medical Center Patient Service Area (PSA). The communities include: Bartholomew, Benton, Boone, Brown, Carroll, Cass, Clay, Clinton, Daviess, Decatur, Fayette, Greene, Hamilton, Hancock, Hendricks, Henry, Jennings, Johnson, Knox, Lawrence, Marion, Martin, Monroe, Montgomery, Morgan, Owen, Parke, Pike, Putnam, Randolph, Rush, Shelby, Sullivan, Tippecanoe, Union, Vermillion, Vigo, and White. The contractor shall provide all labor, supervision and management necessary to carry out the services described herein. This may include the ability of a contractor to subcontract with other facilities that meet state and Medicare accreditations. The comprehensive network-based home health care services shall include, but are not limited to: Specialty Registered Nurses, Licensed Practical/Vocational Nurses, Registered Dietitians, PhysicalTherapist, Nurse/Health Aides, and Homemaker. Must comply with JCAHO Home Care Standards. Seven days/week 24 hours/day coverage would be required but it is expected that most visits will be accomplished Monday through Friday between the hours of 7:30 a.m. to 6:00 p.m.. Delivery of Services: The contractor would assess all patients within a maximum of 48 hours after notice of referral unless the CBEC coordinator specifies otherwise. The contractor would also be responsible for developing, reviewing, evaluating and revising the plan of care as indicated by patient needs. The Standard Industrial Classification (SIC) code is 8082.
