(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format listed in FAR Subpart 12.603 as supplemented with additional information included with this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) Solicitation DAHA3399T6069 is being issued as a Request for Quote (RFQ). (iii) This solicitation incorporates clauses and provisions in effect though the Federal Acquisition Circular (FAC) 97-11. (iv) The Standard Industry Classifications (SIC) for this acquisition are 1422 (Crushed and Broken Limestone) and 2951 (Asphalt Paving Mixtures and Blocks) with the Small Business Size Standard of 500 employees each. (v) Line Item # 1 is for 1300 Tons of Limestone #304; Line Item #2 is for 800 tons of Crushed Limestone #2; Line Item #3 is for 150 Tons of Asphalt Concrete #402 and Line Item #4 is for 150 Tons of Asphalt Concrete #404. (vi) The # 304 limestone and # 2 crushed limestone are required as a sub base for a training runway for the 200th RHS Ohio Air National Guard. Both the #304 limestone and #2 crushed limestone will be stockpiled with dump locations determined by government engineers. The asphalt concrete #402 and #404 are required for surfacing a training runway for the 200th RHS Ohio Air National Guard. Asphalt concrete # 402 and # 404 shall not exceed 20% by volume the amount of recycled asphalt used. The asphalt concrete #402 and #404 will be off-loaded into a government owned paving machine, which will heat it and spread it. (vii) All deliveries shall be FOB. The # 304 and # 2 crushed limestone must be delivered in a one week time frame beginning 10 May 1999 and ending no later than 12 May 1999. The contractor will supply and haul # 304 limestone and # 2 crushed limestone to the NASA Plumbrook, Sandusky, Ohio using the Mason Road gate as the entry point. The asphalt concrete #402 and #404 must be delivered in one day. Asphalt concrete must be delivered either 13 or 14 May 1999. The contractor will supply and haul the asphalt #402 and #404 to the NASA Plumbrook Station, Sandusky, Ohio using the Mason Road gate as the entry point. All deliveries for asphalt concrete #402 and #404 and limestone #304 and #2 will be delivered between the hours of 8:00 a.m. -- 3:00 p.m. Monday thru Friday. Delivery will be coordinated with SSgt. Kyle Lipper @ 419-635-2785 to ensure base engineers are on sight for all deliveries. The government at the place of delivery will make final inspection and acceptance of supplies. The contractor shall furnish delivery tickets in four (4) copies of each delivery made. Tickets shall contain the following information: a) contractor's name and address; b) delivery date; c) quantity delivery with unit and total prices; d) base engineer or designated representative will sign each delivery ticket and provide two (2) copies to the base contracting office. (viii) Provision 52.212-1, Instruction Evaluation -- Commercial Items applies to this acquisition. (ix) The provision 52.212-2, Evaluation, Commercial Items applies to this acquisition. Evaluation will be based on cost and the ability to meet the requirements. (x) Offerors shall include a completed copy of the provision at 52.212.3 Offeror Representations and Certifications -- Commercial Items, with its offer.
