RFP 670-28-99 is issued to provide primary care services at a location in the County of Cayuga in the State of New York. The Veterans Affairs Healthcare Network Upstate New York (VAHNUNY) desires to enter into a contract for Community Based Primary/Preventative Care services for veterans. The resulting contract will be for a period of one year with two options to commence on or about July 1, 1999. A preproposal meeting will be held on Monday, May 3, 1999. The Contractor will be responsible to furnish medical facilities, clinical providers, equipment, supplies, routine radiology and administrative functions to fulfill the level of care specified in the RFP. Service will be at an all-inclusive per visit rate for an estimated 1,800 visits per year. The SIC code is 8011 and the Commercial Items solicitation is unrestricted.
