for commercial items prepared in accordance with the subparts in
Federal Acquisition Regulation 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; bids are being requested and a written solicitation
will not be issued. Solicitation number Request For Proposal (RFQ)
673-73-99 is assigned and shall be referenced on any proposals. This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-11. The Standard
Industrial Classification Code is 3841 and the business size standard
is 500 employees. The James A. Haley Veterans Hospital has a
requirement for it's new Brevard Outpatient Clinic located at 2900
Veterans Way, Melbourne, FL 32940 for ophthalmic instruments and
devices. Specifically, the required items are: (all on a Brand name or
equal basis) Topcon SL-3E Elite Slit Lamp; "K" Tray and screws
(generic is fine); Topcon VT-10 Minus Cylinder Vision Tester, Black;
Haag-Streit R900 Tonometer with two prisms; Front Surface Mirror Set,

operator; ALL OF THE PREVIOUSLY MENTIONED ITEMS AS SEPARATED BY THE
SEMI-COLON HAVE A QUANTITY OF 2 EACH. THE FOLLOWING ITEMS REQUIRE A
QUANTITY OF 1 EACH: Tonopen; Kowa Sl-14 Hand Held Slit Lamp
w/magnification from 10x-16x compact power supply; Kowa Sl-14 Battery
Pack; Standard Opticokinetic Drum (striped); Polaroid Macro 5 SLR
Camera. Proposals are to be on an all or none basis. The government
contemplates one award for these items. Proposals shall be submitted on
an "all or none basis." Partial proposals shall be rejected. All
potential offerors shall be able to provide requested items within 30
days of award.. Offerors shall provide a complete set of provisions
with proposal found at FAR 52.212-3, Offeror Representation and
Certifications-Commercial Items. Provision and clauses found at
52.212-1, 52.212-4, 52.212-5 and 852.210-77 also apply. Proposals shall
be submitted to the address listed herein or faxed to 813-903-4838 and
directed to the attention of Mick Dubuis (90C). The government shall
not be held responsible for quality of fax transmittals. The
solicitation number (RFQ number) shall be listed on the outside of your
