Provide A/E services including investigative, design and construction
period services for relocation and expansion of the medical center's
Physical Medicine, Rehabilitation (PM&R), and Prosthetics services.
Relocation will take place within existing building two ( bldg. 2)
space. PM&R and Prosthetics are currently housed in the basement of
bldg. 2 within grossly deficient space and will be relocated to the
second (2nd) floor of bldg. 2. Bldg. 2, 2nd floor A and C wings will
house relocated PM&R and a portion of the B wing space will house the
relocated Prosthetic service. The anticipated gross square foot of
design space is 29,000 square feet. Oral Surgery and dental operations
currently housed within the B wing will remain and require attention
to egress conditions. Design will incorporate demolition and asbestos
abatement of existing space and complete renovation of space with all
support utilities, services and systems to provide fully furnished and
functional PM&R and Prosthetics suites. Professional design
disciplines required will include, but are not limited to,
Architectural, Structural, MEP, Certified Industrial Hygiene, and
specialized fire and safety engineers or consultants. Work will include
completion of an investigative report stating recommended
modifications, development of conceptual layouts including budgetary
cost estimates, and preliminary scheduling assuring adherence to
budgeted amount and the time frames. Following accepted preliminary
design A/E will provide detailed construction documents, cost estimate,
and scheduling. The A/E firm will establish design through interface
with user group(s). Construction documents will include demolition and
asbestos removal along with interim life safety requirements to be
followed during construction. It is anticipated that construction will
be accomplished primarily with North Texas Health Care System (NTHCS)
construction forces, with certain items such as HVAC systems being
sub-contracted. Items subcontracted will require complete contract
documents fully delineated from primary work and suitable for issue of
separate solicitation for bid. The A/E will provide detailed fire and
sprinkler system construction drawings which when installed are fully
certifiable under current NFPA, City of Dallas, and VA criteria. The
fire sprinkler system must be fully compatible with existing medical
center wide Cerberous Pyrotronics system. All HVAC systems and controls
must be compatible with existing Landis and Staffa controls systems.
Interface with these companies, to ensure compatibility, will be the
responsibility of the A/E. Design meetings will consist of a project
kick off meeting, preliminary design review meeting, and construction
document review meetings at 35%, 65%, and 95% completion of design.
Three complete sets of review documents will be provided at each
submittal review date. Excluding government review time, design will be
completed within sixty (60) calendar days from receipt of a Notice to
Proceed. Plans are to be produced on AutoCAD 14 for  Windows
NT. Specifications are to be produced in  Microsoft Project
4.0. Upon completion of 100% of construction documents, 15 sets of blue
line drawings and bond specifications along with one (1) set of unbound
specifications, and 1 set of specifications on 3 x 5" disk will be
provided. As Built drawings will be provided on Mylar and 1 set on 3.5"
disks. Construction cost limitation is two million and seven hundred
dollars ($2.700,000). Construction period services will include as
built drawings and may include, if needed, but not limited to, shop
drawings, submittal review, and site visits. Shop drawings, submittal
review and site visits would pertain primarily to sub-contracted work.
A/E selection will be based on the following criteria and maximum
point values are shown to reflect relative importance: (1) 50 points
past performance which exhibits specialized experience and technical
competence of work to be performed, experience with hospital renovation
of patient area required, GSA procurement experience and a knowledge of
VA specifications preferred; (2) 50 points experience which exhibits an
ability to design within budgeted cost limitation and preparation of
accurate and highly detailed construction and furnishings cost
estimates; (3) 40 points- capacity to accomplish work in the required
time frame (60 calendar days as stated above); (4) 40 points- quality
control methods used to ensure technical accuracy of work; (5) 40
points- professional qualifications of staff and consultants; (6) 20
points experience and capabilities using CADD disk for submissions, (7)
20 points- demonstrated success in prescribing the use of recovered
materials and achieving waste reduction and energy efficiency in
facility design, and (8) 10 points- geographic location of A/E firms
and consulting servicing offices. In accordance with PL 100-656, this
acquisition is not restricted to business size. Standard Industrial
Classification (SIC) code is 8712 Architectural Services applies to
this acquisition and the small business size determination is NTE $2.5
million in average annual receipts during the preceding 3 fiscal
years. A/E firms which meet the requirements in this announcement are
invited to submit (1) a Standard Form (SF) 254, "Architect-Engineer"
and Related Services Questionnaire, and (2) SF 255 "Architect-Engineer
and Related Services Questionnaire for Specific Project". Submissions
shall be marked to the attention of David L. Hill (90C) at the above
address, and shall be received on or before 4:30 p.m. local time,
