synopsis/solicitation for commercial items prepared in accordance with
the format FAR subpart 12.6 as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. Solicitation No. RFQ 656-22-99. This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular (FAC) 97-10. SIC code
8011 applies. SectionC -- DESCRIPTION/SPECS./WORK STATEMENT 1.
Responsibilities and duties: a. Contractor to provide a board certified
Internal Medicine Physician to provide medical primary care services in
the St. Cloud VAMC Community Based Brainerd Outpatient Clinic (Brainerd
Regional Human Service Center, State of Minnesota, 1777 Highway 18
East, Brainerd MN 56401). Initial coverage will be provided at 20 40
hours per week while permanent physician recruitment is completed. Once
permanent VA Staff are assigned, the contractor will provide ongoing
flexible coverage for sick leave and annual leave by the staff
physician or physician extender. Clinic hours of operation are 8:00 am
4:30 pm Monday through Friday except on Federal Holidays. The ten
holidays observed by the Federal Government are: New Year's Day, Martin
Luther King's Birthday, Washington's Birthday, Memorial Day,
Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving,
Christmas or any other day specifically declared by the President of
the United States to be a national holiday. b. The Contractor will
provide consultative medical services to the physician extenders
working in the clinic. c. The Contractor will answer emergency calls
within 30 minutes. d. The services to be performed by the contractor
will be performed within the VA policies and procedures and the
regulations of the medical center staff bylaws of the VA facility. e.
The St. Cloud VAMC will provide equipment for the routine care
provided. f. The Contractor will provide timely, electronic
documentation and signature for all visits. g. The physician will
cooperate and participate in quality improvement activities. The
results of all Quality Improvement activities performed by the
contractor involving VA patients will be shared with the VA. This will
include but not be limited to quality improvement plans, indicator
data reports, report of findings, conclusions, recommendations and
written plans for action taken in response to such conclusions and
recommendations. h. The Contractor will use managed care principles to
assist Medicine Primary Care in identification of areas to improve
access and ensure appropriate utilization of services within the VA
system. i. The Contractor shall be responsible for protecting the
personnel furnishing services under this contract. To carry out this
responsibility, the Contractor, shall provide the following for these
personnel: Worker's compensation, professional liability insurance,
health examinations, income tax withholding, and social security
payments. The parties agree that such personnel shall not be considered
VA employees for any purpose and shall be considered employees of the
Contractor. j. The services specified may be changed by written
modification to this contract. Upon agreement with the Contractor, the
modifications will be prepared by the VA Under Secretary for Health or
designee. k. Other necessary Personnel for the operation of the
services contracted for at the Brainerd Community Based Outpatient
Clinic will be provided by the St. Cloud VA Medical Center at levels
mutually agreed upon which are compatible with the safety of the
patient and personnel and with quality medical care programming. l. The
services to be performed by the contractor will be under the direction
of the Chief of Staff or his designee, VA Medical Center, 4801 8th
Street North, St. Cloud, Minnesota 56303. 2. Internal Medicine
Qualifications: a. Documentation shall be provided to prove the
individual(s) is Board Certified to perform procedures outlined in this
section under item 3. b. The provider shall have an unrestricted
license to practice medicine in one of the states or territories of the
United States or District of Columbia. c. The provider will be Code
Blue ACLS trained. d. The individuals performing under this contract
shall meet all requirements for appointment to the Medical Staff, VA
Medical Center, St. Cloud, MN as outlined by the Medical Staff Bylaws.
e. The Physicians assigned by the contractor to perform procedures in
the VA must go through the VA Credentialing process for delineation of
clinical privileges. f. The individual(s) performing services must
possess basic proficiency in spoken and written English in accordance
with Public Law 95-201. 3. Procedures: a. The physician will provide
office based medical primary care model which focuses on preventive
health and chronic disease management. b. The physician will be
responsible to stabilize and treat active illness and facilitate
hospital admission when necessary. c. Hospitalizations and specialty
referrals will be directed to the appropriate VA facility whenever
medically possible. Emergent care will be coordinated within the
community through fee basis services. d. The physician will be
responsible to maintain follow-up after admission and specialty clinic
referrals to provide continuity of care. e. The physician will
cooperate and participate in quality improvement activities. f. The
physician will participate in clinic meetings and cooperate with
processes concerning access, quality, cost and productivity. g. The
physician will work with other clinic staff to provide a team approach
to offer complete and comprehensive care to each veteran. h.
Diagnostic codes will be assigned according to the guidelines outlined
in the ICD-9-CM coding system. i. Procedure codes will be assigned
according to the guidelines outlined in the American Medical
Association Physician's Current Procedural Terminology, as updated
annually. j. A typical clinic day (8:00 4:30 pm) will allow for
approximately 18 20 patients to be scheduled in 20 minute appointment
slots. There will be time set aside in the schedule to accommodate
walk-in/urgent appointments needing services same day. 4. Point of
Contact: a. A VAMC technical representative of the Contracting Officer
will be designated to represent the contracting officer in furnishing
technical guidance and advice or generally supervise the work being
performed under this contract. The foregoing is not to be construed as
authorization to interpret or furnish advice and information to the
contractor relative to the financial or legal aspects of the contract.
Those matters are the responsibility of the contracting officer and
shall not be delegated. 5. Monitoring of Contractor's Performance a.
The contractor is paid on a monthly basis visit for hourly services
performed in accordance with the prices/costs of the contract. Hours of
service will be documented on a clinic schedule and verified by the
clinic manager. b. Every procedure performed is entered into a computer
database. The Veteran's identification number, source of referral,
diagnosis, and line items performed by the contractor are entered into
the database software. The invoice certifying official shall ensure
review of the database that the contractor has performed the visits
required. c. The Government may evaluate the quality of professional
and administrative services provided, but retains no control over the
medical or professional aspects of services rendered (e.g.,
professional judgements, diagnosis for a specific medical condition).
6. Payment a. The VA agrees to reimburse the contractor on an hourly
basis for services furnished at the price listed the Schedule of
Supplies/Services of this contract. b. Sums due the contractor will be
paid monthly in arrears upon receipt of a properly prepared invoice
submitted by the contractor. c. Bills rendered by the contractor to the
VA for services furnished a VA beneficiary under the terms of this
contract shall be in full. Neither the beneficiary, his insurer, nor
any other third party shall be billed by the contractor. 7. Terms of
Contract a. This contract is effective for one year from date of award
with two pre-priced option years. The contract is subject to the
availability of VA funds. M -- EVALUATION FACTORS FOR AWARD Award will
be made to the offer whose proposal is most advantageous to the
Government, price, and other factors considered. The evaluation
factors, listed in descending priority are as follows: 1. Personnel
Qualifications: Provide licensure, ACLS certification, Quality
Assurance Indicators, and personal references. 2. Cost: The cost
proposal will be furnished on section B (Supplies or Services and
Prices/Costs). 3. Past Performance: Provide references where similar
services have been provided. 4. Availability of Service: Describe your
flexibility of hours and providing additional services if the need
arises. The following provisions apply to this solicitation; 52-212.1
