C -- RENOVATE MORGUE AREA DUE 052499 POC CAROLYN THOMAS, (414) 902-5417, Contracting Officer Provide professional engineering services to develop construction documents to renovate the Morgue Area located in the first floor of the Hospital Building, VA Chicago Healthcare System, West Side Division, 820 South Damen Avenue, Chicago, IL 60612. The work includes, but is not limited to, installation of new dedicated ventilation system to meet current code requirements, provide ventilated storage cabinets for formalin fixative and for containers with specimens in formalin outside the main morgue area, construction of new epoxy coated walls, new chemical resistant flooring and epoxy coated ceiling with continuous diffuse lighting in main morgue room and new autopsy and dissection tables. The project will also include construction of a viewing area outside the main morgue room appropriately decorated for the families of the deceased. Estimated construction cost is between $250,000 and $500,000. The area of consideration is restricted to 100 miles of the VA Chicago Healthcare System, West Side Division, Chicago, IL. Submit two (2) copies each of SF 254 and SF 255 with appropriate data. Selection criteria will be based on(1) Past performance on similar projects, maintaining project schedules and project budgets; (2) Overall design experience and capability with architectural and civil projects; (3) Qualifications of staff to be assigned, with special emphasis on field personnel to be utilized; (4) Cost control effectiveness; and (5) Ability of A/E to produce specifications on a 3.5 disk for microsoft Word format release 7.0., ability to produce drawings on a CAD system fully compatible with Auto CAD Release 13 format on 3.5 disk. Ability to produce hard copies of cost estimates. Review and approve all submittals. Provide two(2)job inspections(other than final inspection)as requested by the Contracting Officer's Technical Representative. Conduct a final inspection and deliver a report within 10 days.
